Informed Consent and Acknowledgement
I hereby give my approval for all activities prepared by Playmaker London. In exchange for the acceptance I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Playmaker London and all its respective officers, agents, and representatives from any and all liability for injuries arising out of traveling to, participating in, or returning from activities conducted by Playmaker London.
In case of injury I hereby waive all claims against Playmaker London, including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including football.
I give my consent that if an emergency medical situation arises, Playmaker London may act in loco parentis for administration of first aid and/or other medical treatment that in the opinion of a qualified medical practitioner may be necessary. I also understand that in such circumstances all reasonable steps will be taken.
I give permission for my photograph and video to be used by Playmaker London for promotion across the club’s website and social media pages, display purposes, other printed publications and training or analysis purposes.
Playmaker London will take all steps to ensure these images and videos are used solely for the purposes for which they are intended. If you become aware that they are being used inappropriately, please inform us immediately.
I give consent that Playmaker London may contact me to inform and update me regarding any reasons required by the club aswell as offers and promotions.
Playmaker London are committed to protecting and respecting your privacy. For any personal data you provide for the purposes of your registration, Playmaker London is the Data Controller and is responsible for storing and otherwise processing that data in a fair, lawful, secure and transparent way. Details on this form will be held securely and will only be shared with coaches or others who need this information in order to meet specific needs.
