TATTOO SUBMISSION FORM

Thank you so much for considering me for your next project! Below you'll find questions to help me best assess your needs, and understand the scope of the project so I can make accurate estimates of time and pricing. Please fill this form out to the best of your ability, and read all the information linked at the bottom that shows pricing, scheduling, and portfolio.

Once we have received your submission, you will automatically be considered for the next booking period, which is October-December 2026. If we have further questions, we will email you from an address with a lowbarart.com domain! Consult invites will be sent in September, and if I run out of room, you will be added to the Winter 2027 waitlist and be contacted in November or early December :)

PLEASE INCLUDE A PHOTO OF THE AREA YOU WOULD LIKE TO GET TATTOOED. Do not take the tattoo in the mirror, so we can avoid confusion. Have a buddy take the photo from 1-2 feet away, and make sure the body part is parallel to the lense to ensure proportional accuracy. If this is a wrapping piece (I.e. sleeve) include multiple sides of the area.

Luka uses standard precautions, so is prepared to tattoo you REGARDLESS of status. We want to know only so we can best accommodate your needs! Mango or vitamin D allergy? Endo? Blood pressure? Diabetes? HRT? All of these and more can affect your experience!
