I, the undersigned, being aware of my own health and physical condition, and having full knowledge that my participation in any exercise, artistic, or recreational program may be injurious to my health, am voluntarily participating in such activity with NobleMotion Dance.
Having such knowledge, I hereby release NobleMotion Dance, their directors, officers, employees, and representatives from any and all liability for injury or illness that may incur as a result of participating in the NobleMotion Dance Summer Dance Intensive, June 23rd-June 28, 2025. I hereby assume all risks connected therewith and consent to participate in said program.
I understand that NobleMotion Dance does not have medical personnel available at the Summer Dance Intensive and I grant NobleMotion Dance permission to authorize emergency medical treatment, if necessary. I understand that NobleMotion Dance assumes no responsibility for any injury or damage that might arise out of or in connection with such authorized emergency medical treatment.
I agree to disclose any physical limitations, disabilities, ailments, or impairments that may affect my ability to participate in said audition.
