Tournament Sign-Up Form
Tournament Rules:
- Each youth bowler who is in the tournament will be entered in a drawing to receive a chance to win a new bowling ball valued up to $120.00 (one bowling ball for all entrants ages 8-18)
- *SMART Account program is set up by the United States Bowling Congress for youth scholarships. For more information on SMART Accounts go to www.bowl.com
- This is a River Valley USBC tournament and is NOT part of any other tournament event
- **2024-2025 average with at least 21 games must be used OR current 2025-2026 average with at least 12 games
- Handicap is based on 90% of 200. All entrants should check in 30 minutes before their shift
Payment can be made online (below) or by providing the Youth Bowling League Leader a check made payable to River Valley USBC.

$16 Prize Fund/$8 Lineage/$1 Administrative Fees
