  • We are pleased your child/dependent is interested in volunteering with Claire House Children’s Hospice. We hope the experience will be a rewarding, enjoyable and help your child/dependant to gain experience for their future development. Before they can be placed as a volunteer, Claire House Children’s Hospice will require a consent form to be filled out by their parent/guardian. 
    Please note that your child/dependant will not be allowed to be a volunteer unless this form is completed. 

    Please understand the following: 

    1. All volunteers with Claire House Children’s Hospice under the age of 18 must have parental/guardian consent in order to volunteer. 
    2. I understand that photography may be utilised during the volunteering experience. I grant permission to Claire House Children’s Hospice to use my child/dependent’s images in print form. 
    3. I accept the responsibility for my child/dependent’s safety and security and personally undertake to have my child/dependent act in a responsible and safe manner as well as to adhere to the Volunteer code of conduct. 

    Tasks while volunteering within a Claire House shop could include the following, with full training given: 

    • Steaming clothing- using a steamer.
    • Using a tagging gun to attached price tags to clothing. 
    • Working on the shop floor, dealing with enquiries from customers. 
    • Using the till, handing money, serving customers. 
    • Manually handing donations and stock in a safe and responsible manner.  
       

    By completing the information below, I consent to my child/dependent volunteering for Claire House Children’s Hospice.  I understand and agree to all comments noted above.
