I hereby acknowledge that (as per ELFA regulation), I, as well as any guest accompanying me, are strictly prohibited from consuming any alcoholic beverages (whether openly or concealed) next to or near the fields where junior games are being played. This restriction applies regardless of the time of day. I understand that failure to adhere to this rule may result in penalties, including but not limited to, being banned from the club and being liable to pay a fine to the Eastern Local Football Association.
KIT SIZING- kit is slim fit and a small cut, so if you are unsure rather opt for a size bigger size [please note that there will be kit fitting at our Open Day to confirm the size you would like to order. Should it be different than indicated on your form, it can be changed on the day]. Please note RESPONSIBILITY FOR ORDERING THE CORRECT SIZE LIES WITH THE PARENTS. CONDITIONS

If monthly fees are outstanding at any stage, the player's card will be withdrawn and he/she will be unable to participate in matches until the account is brought up to date. If there are any mitigating circumstance please come and discuss directly with the Committee.
All Payments are to be made into the Club's bank account via EFT.

The club Treasurer, Committee Members, Coaches & Managers ARE NOT allowed to receive any monies in the form of cash payments from players/parents for Registration, etc.
FORCE MAJEURE CANCELLATION

In the event that a Government lockdown occurs, as a registered member of OBFC, you will be required to abide by any and all lockdown policies and procedures set out by our local football authorities and the Government.
OBFC will not be held liable for cancellation, suspension, delay or shortening in services resulting from a Force Majeure event, including, but not limited to: field closures, equipment failure, war, epidemics, pandemics, floods, fires or other severe weather conditions.
AGREEMENT

I hereby agree that:

I will inform the coach of any physical disability or ailment that may affect my own safety or that of my child, or any other player.
I will learn the rules of the game and the policies of the league.
I will be a positive role model for other players, spectators and children and encourage sportsmanship by showing respect andcourtesy, and by demonstrating positive support for all players, coaches, officials and spectators at every game, practice or tournament that the Club is involved in. Should I be in disagreement with any official I will never question or confront them at the game field or during the game, and undertake to speak with them in private at an agreed upon time and place. The contents of such discussion will remain private unless either or both parties would like to present these contents to the relevant Chairperson.
I will not interfere in team selections and will respect the autonomy of the coach in this matter.
I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting, refusing to shake hands or using profane language or gestures.
I will never ridicule or yell at any participant for making a mistake.
I will emphasize skills development and practices and how they will benefit a player.
I will refrain from coaching any player during games and practices, unless I am one of the official coaches of the team or asked by the coach to stand in.
I recognize that Coaches are volunteers. They give up their time for me/my child. I will respect them and their decisions.
My child will be on time for all matches unless prior arrangement has been made with the coach or manager. I will inform my child's coach/manager at the time of team selection whether he/she will be available to play the match or not.
I will teach my child to play by the rules and to resolve conflicts without resorting to hostility or violence. (Where applicable)
I will teach my child that doing one's best is more important than winning, so that my child will never feel defeated by the outcome of a game or his performance. (Where applicable)
I will praise my child for competing fairly and trying hard, and make him feel like a winner every time. (Where applicable)
I will always be proud of being part of the Club and uphold its name as a representative.
I will abide and support the constitution of the Club, therefore commit myself to pay my subs and registration fee as I have indicated on the payment options attached.

By registering your child as a member of the club, you also agree that if you fail to abide by the aforementioned rules and guidelines, you the player/parent/guardian will be subject to disciplinary action by the Executive Committee of the Club, which could lead to eventual forfeiture of the right of participation.

I hereby consent and authorize the duly elected officials to act on my behalf during the 2025 football season.
I am aware that neither the Club nor its officials accept responsibility for any loss, injury and/or damage that I or my property/the person or property of my son/daughter may sustain whilst engaging in any football activity during the 2025 season. I waive any right that I and/or my son/daughter may have to claim compensation against the Club or any of its officials/members in respect of any loss, injury and/or damage incurred whilst engaged in any football activity, howsoever arising and whether as a result of negligence or otherwise, and I indemnify them against all claims.
I have read, understood and accept the conduct expected of me as outlined above, and give consent to the club to use my /my child's personal information for the intended use to run the club.
POPI ACT COMPLIANCE :

I understand that Old Bens utilise all personal information obtained to effectively and efficiently run the functions of the club. I also understand that Old Bens endeavour to follow the POPI Act and the POPIA Policy of the club (available on request).
I Hereby confirm that all information provided to Old Bens Football Club with this Registration Form is correct and understand that the Annual Registration Fee, subscription fees and any prior outstanding fees must be paid up to date, in order to be registered, practice and participate in matches for the 2025 season.
