2026 ArMA Awards Nomination Form

The Arizona Medical Association invites you to recognize those going above and beyond in Arizona healthcare by nominating them for an ArMA Health Care Award.

Physicians, residents, fellows, medical students, allied health professionals, healthcare leaders, programs, and organizations are eligible for nomination. Please refer to the award requirements below for more information on eligibility.

We accept nominations year-round in anticipation of our Health Care Awards Ceremony, which will be held in Fall 2026. Nominations submitted before July 17, 2026, will be considered for this year's ceremony.
