Fill out the form carefully for registration. This registration form is for an American Red Cross CPR course. No refunds will be given.
Each participant must fill out this form and pay for the course. The course can be subject to cancellation due to inclement weather, instructor sickness, and lastly to low enrollment. If any of these instances occurs, the instructor will reschedule the class for a later date. All classes are non-refundable but can rescheduled by reaching the instructor at 978-802-5324. Thank you.
