Young Warrior Scholarship Application
This award will be given to local, qualified school age youth with sickle cell disease who are striving to achieve their goals and expand their knowledge through extracurricular activities and/or sports.
Requirements:
1. Must have sickle cell disease or the trait.
2. Must be a high school senior with a GPA of 2.5 or better.
3. Must be a resident of Kentucky or Kentuckiana, which includes all counties in Kentucky and Clark, Floyd, Harrison, Scott, and Washington Counties in Southern Indiana.
4. Must be attending a public school in one of the counties above.
Criteria:
1. Complete this application and submit it with all documentation.
2. Letter of verification from a physician of diagnosis of SCD.
3. Proof of enrollment or letter of acceptance from educational institute for which you are seeking this scholarship.

5. Two references to submit letters of recommendation, one from a teacher and one from another adult such as a counselor, assistant principle, or principle.
6. Personal essay containing at least 500 words. Please describe yourself and tell us how having SCD has affected your life. Include your educational goals, how you expect to achieve them, reason why you think you should receive the scholarship.
Please have all documentation ready when filling out this form. You will be asked to submit your evidence at the end of the application.

All financial assistance will be sent through the school's financial aid office. Disbursement will be provided quarterly or each semester, depending on the institution. The scholarship will be awarded annually continent upon funds. If the applicant has not chosen a trade school, college, or university the application will be put on hold until the educational institution can be verified.
