Statement of Consent & Waiver
I hereby agree and declare that I am the legal parentguardian of the above-named child and hereby consent to the child's participation in the activities that are described in this form or that may otherwise be reasonably expected at an outdoor educational camp. Participation in any program which involves physical activity exposes the camper to certain risks and dangers. Students at this camp will be spending most of their day outdoors in the forest, whether working on crafts, closely exploring nature, or hiking. Accidents and injuries are always a possibility, and it is impossible to foresee and protect the camper from all conceivable dangers. I understand that activities of the kind described may result in physical injury to my child but nonetheless specifically request that he or she be allowed to participate in those activities.
I understand that the activity supervisor will make every effort to contact me in the case of an emergency. If the above-named child requires any emergency medical treatment or procedures during the activities, I hereby consent to activity supervisor taking necessary actions to treat or arrange for such procedures or treatments according to the informed discretion of the activity supervisor. I accept responsibility for the costs of all such medical treatment.
I, the parent/guardian, hereby agree and declare that I have carefully read and understand the scope of the summer camp activities and I consent to the participation of the above-named child to these activities.
Camp management cannot be held responsible for any harm that may occur to the child without the fault of the camp management or activity supervisors. By signing this Waiver and Release of Liability, with full appreciation of the risk involved, I hereby voluntarily release and forever discharge the camp and activity supervisors from any and all legal or financial responsibility for any personal injury, disability, illness, damage, medical expense or death, arising from or related to my child’s participation in Summer Camp. I agree not to make any type of legal or equitable claim with respect to any injury my child may suffer, whether or not it arises through the negligence, omission, default or other action of anyone affiliated with the camp, including other campers. I further agree that if any such claim is made, I will indemnify the camp and/or activity supervisors with respect to any such claim, injury or damage.
