  • Media Release

    I give permission for still or video pictures, as well as written statements, of my child or myself to be used for MATE promotional purposes.  These efforts include, but are not limited to brochures, flyers, slideshows, email newsletters, newspaper articles, and websites.

  • Risk and Release

    I understand that there are inherent risks involved in any mission event and that I am volunteering for an event that includes the repair of damaged and/or potentially unsafe buildings. I understand that MATE and its partners do not guarantee my safety, nor do they claim responsibility for my safety or personal belongings. I/we hereby assume all risks related to the injury, illness, loss, or harm that may occur to me/my child during or after the duration of the event. I release and hold harmless MATE (to include its partners, officers, directors, staff, affiliates, and successors) in this mission event from any and all liabilities for injury, illness, death or loss/damage of property that may occur during the course of my own or my child’s involvement, including all travel during the event. The validity, construction, and interpretation of this Release of Claim shall be governed by and construed in accordance with the laws of the State of Maine exclusively, and that any and all claims arising from and/or related to this agreement or my participation in a Mission at the Eastward home improvement project may only be brought in the courts of competent jurisdiction of Franklin County, Maine.

  • Medical Waiver and Release
    In the event medical treatment is required from a physician and/or hospital personnel I agree to hold such person/s and MATE free and harmless of any claims, demands, or suits for damages or injuries arising from the giving of such consent. The volunteer (and parent) releases and forever discharges MATE (to include its partners, officers, directors, staff, affiliates, and successors) from all and any claims that may arise due to first aid or decisions regarding medical care. The validity, construction, and interpretation of this Release of Claim shall be governed by and construed in accordance with the laws of the State of Maine exclusively, and that any and all claims arising from and/or related to this agreement or my participation in a Mission at the Eastward home improvement project may only be brought in the courts of competent jurisdiction of Franklin County, Maine.

  • Protection of Children

    The State of Maine’s Child and Family Services Child Protection Act requires that if you are aware of any potential child abuse situation, be it physical, emotional, sexual, or neglect, you must report it to the Maine Department of Health and Human Services (DHHS) at 1-880-452-1999, or to Chris DeLisle, MATE’s DHHS Designated Agent, at 207-557-5119.
