Curious Campers Nature School is seeking your permission to take photos and videos of your child for exclusive use in marketing materials, which may include:

• Curious Campers Nature School website
• Curious Campers Nature School social media accounts
• Curious Campers Nature School newsletters and emails
• Marketing collateral for Curious Campers Nature School (e.g., posters, brochures, flyers, etc.)
• Advertising materials for Curious Campers Nature School (e.g., magazine or newspaper articles, other printed media, etc.)

Liability Form
The undersigned(s) being the lawful parents) and/or guardian(s) of the above child hereby consent to the child’s participation in all activities conducted by Curious Campers Nature School and to the child’s participation in all events related to said activities.

The undersigned(s) hereby further authorizes) emergency transportation
by either the center staff or, if necessary, by ambulance or other emergency vehicle. If there is no medical emergency, the staff will first use reasonable efforts to contact the parents) and/or guardians) before administering or authorizing any treatment.

Curious Campers Nature School, being well child-proofed and with constant supervision, ensures the safety of all children. However, accidents do happen. The undersigned(s) assume(s) all risk of injury or harm to the child associated with participation in the childcare and agree(s) to release, indemnify, defend and forever discharge Curious Campers Nature School and its staff, employees, and agents of and from all liability, claims, demands, damages, costs, expenses, actions and causes of action in respect of death, injury, loss or damage to the child, or by the child, howsoever caused, arising or to arise by reason of or during the child’s participation in the center.
