  • Thank you for entrusting Fable Fairy Tax Service with your private information. We promise to always to give our absolute best to serve you. To begin our partnership, we ask you to complete this Organizer before your appointment. If you’re a returning client, please use the proforma Organizer already provided to you.


    This Organizer consists of 27 sections designed to gather the information we need to serve you best. Please provide scans of the requested documents in PDF format only as the IRS WILL NOT accept anything else. Photos or screenshots are not allowed, as they may slow down the process of helping you.

     
    As you complete the Organizer, keep the following in mind:

    • If a question or section doesn’t apply to you, simply skip it and move on.
    • If you need to locate documents we’ve requested, save your progress, gather the necessary paperwork, scan them to create PDFs, and then continue from where you left off. Please avoid skipping documentation requests, as doing so may also delay the process.
    • You must SAVE your work with a valid email address in order to come back and edit the form you are about to begin. Click the next button, enter your name and email, then click the Save button to get the link in your email.

  • Client Payment Agreement

  • By signing this intake form, the Client agrees to pay all software fees and preparer fees to Fable Fairy Tax Services exclusively through the company’s designated bank partner. The Client acknowledges that any unauthorized alteration, cancellation, or forced change to the agreed payment method (e.g., circumventing the bank partner’s process or redirecting funds outside the approved system) constitutes a material breach of this agreement.

    In the event of such a breach, Fable Fairy Tax Services reserves the right to:

    •  Pursue all legal remedies available to recover unpaid fees, including litigation, wage garnishment, bank levies, liens, asset seizure or any other means necessary;

    •  Terminate all current and future services immediately without notice;

    •  Hold the Client liable for all collection costs, including attorneys’ fees, court costs, interest, and penalties.

    The Client further agrees that disputes regarding payment methods must be resolved in writing with Fable Fairy Tax Services prior to any changes. Failure to adhere to the approved payment process voids all service agreements irrevocably.
