I, the undersigned, grant permission for my child’s individual and/or team photograph to be taken and used by Kanaijs Photography for any and all purposes. I, the undersigned, give my permission for my child’s photo to be used byKanaijs Photography for the following purposes:
- League publications Promotional materials (posters, flyers, brochures)
- School website
- Social media accounts (Facebook, Instagram, etc.)
- Media coverage (local news, league promotions)
I understand that my child’s photo may be used for these purposes and acknowledge that no compensation will be provided for the use of the image.

- Uniform is clean and shirt is tucked in.
- Proper baseball shoes are worn.
