2025 Scholarship Application

Who May Apply: Must be a senior female in the Oklahoma City Metropolitan area.

Gather all information before starting your application (as all fields are required Select N/A if it does not apply. You must answer each question on this application. Your complete application should include the following:

A typed autobiographical sketch of at least 250 words, but not more than 300 words, including why you need the Delta Scholarship(s) to attain your goal.
A copy of a seven-semester official transcript which includes your GPA.
Minimum GPA of 2.75 needed to apply.
Two (2) letters of recommendation from a non-relative. The recommendation may be from school personnel, church, a civic organization, or an employer.
A headshot photograph of yourself (can be taken on your phone, with a solid background).
A copy of Page 1 and 2 of your parent(s) or guardian(s) IRS Form 1040 from 2023 with SSN blacked-out.
Copies of W-2 Forms from your parent(s) or guardian(s) for 2024 with SSN blacked-out.

If parents are self-employed with no W-2, a copy of prior year (2023) and current year (2024-estimate if not yet filed) of Schedule C and/or Schedule E or F with SSN blacked

(Tax forms will be destroyed after selection).

If no return required, provide other proof of income.

To be awarded the scholarship: Applicant must provide verification of enrollment. If selected scholarship recipient will be required to attend the Breakfast for Milady Event on May 10, 2025.

Applicants may apply to all scholarships for which they qualify.
