PUBLIC LIABILITY INSURANCE

The Insurer will issue an Insurance Policy for each insured person with Total Sum Insured in one package covering with automatically by request :

- The participant should be minimum 05 years old and maximum 65 years old

1. DEATH: IDR 500.000.000,00 a.o.p
2. PERMANENT DISABLEMENT MAX. : IDR 500.000.000,00 a.o.p
3. MEDICAL EXPENSES MAX. : IDR 50.000.000,00 a.o.p
4. AMBULANCE EXPENSES MAX. : IDR 2.000.000,00 a.o.a

Total Limit for the whole period IDR 5.000.000.000,00 Combine single limit. 

PARTICIPANT WAIVER

Participant Waiver: I hereby voluntarily waive, release, forever discharge, and agree to hold harmless and indemnify Surya Bintang Adventures, its agents or employees, and all other persons or entities from any and all liability, claims, demands, actions, or rights of action, which are in any way connected to my participation in the activity for any injury, death, illness, or disease, injuries to myself, and damage to my property, other than a legitimate personal accident policy afforded by Surya Bintang Adventure as a result of my own injuries sustained during the activity. 

DISCLAIMER / RELEASE OF LIABILITY: I agree that I assume all risk of injury, loss, or damage during participation in Surya Bintang Adventures (Rafting, Cycling, Trekking, Swing, Touring/ATV, etc.).

I hereby agree that Surya Bintang Adventures is not responsible for any injury, loss, death, or damage. This release of liability and assumption of risk agreement is based on the following conditions:
1. I am 5 years old or under, or 65 years old or over.
2. I am taking medication or I am under the influence of drugs or alcohol.
3. I have a physical condition which might affect my safety or health while joining an activity (for example: heart problems, asthma, recent broken bones, etc.).
4. I am pregnant.
5. I am currently under doctor's care.
6. I may have difficulty walking down or up the steps to or from the river (approximately 10-20 minutes walk each way).

I, the undersigned, hereby acknowledge that I am the Person in Charge (PIC) or representative of the group and that I have voluntarily chosen to participate in Surya Bintang Adventures. I understand that certain risks are inherent in any recreational activity and cannot be eliminated, altered, or controlled. These risks contribute to the unique character of the activity and can also be the cause of physical or mental injury, illness, death, loss, or damages, and also the damage or loss of personal property.

In signing this document, I fully recognize and understand that if I (or a minor on whose behalf I am signing this release) get hurt or die, I, my family, relatives, and/or acquaintances would be giving up the right to make a claim or file a lawsuit against Surya Bintang Adventures, its employees/staff, or the booking agent (person/company).

I understand and acknowledge that the activity I am about to voluntarily engage in as a participant bears certain known and unanticipated risks which could result in injury, death, illness, or disease, and physical damage to my property and belongings.
