New Student Application Form

Please take a few minutes to read through the MOST Handbook before applying. 

Failure to submit documentation or presenting misinformation for expenses will jeopardize your scholarship(s).

You must be able to show proof of your financial responsibility for every person in your household, either by showing him or her on your tax return, on an official document of public assistance (Families First, SSI, food stamps, etc, or on your receipt of child support).

1. Earned Income
   1. All tax filers must submit a SIGNED AND FILED copy of the 2025 Form 1040 tax return, including page 2, (plus Schedule C, if self-employed) and all employee-issued W-2s and/or 1099s.
   2. If anyone in your household earned income that was NOT reported on a tax return, submit some sort of documentation (e.g. letter from employer, Disability Statement, deposits to bank account, etc.) showing 2025 income.
2. Food Stamps: Food stamp Issuance History for calendar year 2025. (Contact your caseworker if you do not have a copy.)
3. Families First: Cash Issuance History for calendar year 2025. (Contact your caseworker if you do not have a copy.)
4. Social Security Income: 1099 Forms from Social Security Administration for each person receiving benefits.
5. Child Support: 
   1. If support is court-ordered, provide a Custodial Parent Payment Summary. (If you have your Case ID or Member ID, you can print a summer at https://apps.tn.gov/tcses-app/login.html.)
   2. If child support is not court-ordered, have the paying party sign a statement of amount paid in 2025.

I certify that all the information provided on this form is true and complete. I understand that by presenting misinformation will result in the permanent loss of my family's scholarship(s).

I agree to:

• Ensure my child(ren) is in school on the first day of classes;
• Fulfill my financial obligations to my child(ren)'s school account(s) in a timely manner;
• Commit to at least 90% attendance by my child(ren);
• Sign all required MOST documents in a timely manner; and,
• Comply with the standards of my child(ren)'s chosen school(s).

I understand that I understand that failure to comply with these statements will result in the permanent loss of my family’s scholarship(s). I also give permission for my child(ren)’s school(s) to release information about grades, test scores, and any awards/honors my child(ren) receive(s) to MOST or any agency authorized by MOST for the purpose of conducting evaluations of the program. Finally, I agree to release MOST from any liability in its efforts to provide this/these scholarship(s).
