Failure to submit documentation or presenting misinformation for expenses will jeopardize your scholarship(s).
You must be able to show proof of your financial responsibility for every person in your household, either by showing him or her on your tax return, on an official document of public assistance (Families First, SSI, food stamps, etc, or on your receipt of child support).

I also give permission for my child(ren)’s school(s) to release information about grades, test scores, and any awards/honors my child(ren) receive(s) to MOST or any agency authorized by MOST for the purpose of conducting evaluations of the program. Finally, I agree to release MOST from any liability in its efforts to provide this/these scholarship(s).
