COVID TEST REQUIREMENT - Please wait to fill this form out until after you have tested, please! - During cold and flu season we require covid tests if you choose not to wear a mask. Please remember that this test must be taken the night before, and no later than 45 minutes before your appointment to avoid incurring late charges. This test must be taken BY YOU ONLY, and needs to have a date and time stamp (this can be done by holding a phone, watch, tablet, or computer display showing the date and time). [We apologize we have to do this, but we have had a couple people try and use the same exact photo / old photos of covid tests just to get out of taking one.] --- MASK OPTION: If you choose to wear a mask instead of testing, please simply take a photo of your mask so we know what to expect. I acknowledge by signing this agreement that I have been given the full opportunity to ask any and all questions which I might have about the obtaining of a tattoo and that all of my questions have been answered to my full satisfaction. If I have any condition that might affect the healing of this tattoo, I will advise my tattoo artist. I am not pregnant or nursing. I am not under the influence of alcohol or drugs. I understand that the studio can & will ask me to leave if I appear OR smell of alcohol, marijuana, or other substance AND I WILL BE FINANCIALLY RESPONSIBLE FOR PAYING THE FULL AMOUNT OF TIME OF MY SCHEDULED APPOINTMENT.

I do not have medical or skin conditions such as but not limited to: acne, scarring (Keloid) eczema, psoriasis, freckles, moles or sunburn in the area to be tattooed that may interfere with said tattoo. If I have any type of infection or rash anywhere on my body, I will advise my tattoo artist. Please remember that we can not tattoo over any suspicious marks on the skin, nor can we tattoo burnt skin. If you have a scar that we are unaware of near the area you want to get tattooed, we can not tattoo fresh wounds and ask you to allow 6-12 months for this to heal before having work done.

I acknowledge it is not reasonably possible for the representatives and employees of this tattoo shop to determine whether I might have an allergic reaction to the pigments or processes used in my tattoo, and I agree to accept the risk that such a reaction is possible.
I acknowledge that infection is always possible as a result in obtaining a tattoo, particularly in the event that I do not take proper care of my tattoo. I have received aftercare instructions and I agree to follow them while my tattoo is healing. I agree that any touch-up work needed, will be done at my own expense. I understand that I am responsible for purchasing aftercare, and proper care of my tattoo. I realize that variations in color and design may exist between any tattoo as selected by me and as ultimately applied to my body. I understand that if my skin color is dark, the colors will not appear as bright as they do on lighter skin tones. Again, the final color depends on my diet, health, and how I care for the tattoo when I leave.

I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my tattoo.

I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change or remove my tattoo. To my knowledge, I do not have a physical, mental or medical impairment or disability which might affect my well being as a direct or indirect result of my decision to have a tattoo. I am of sound mind and judgement and am voluntarily paying for this appointment/ service.

I understand that my deposit (if I paid a deposit) will come off the total in the end of my tattoo. If my tattoo is not completed the day of the appointment and must come back for another visit, my deposit may be deducted at that time. Should I choose to cancel any last appointments, the deposit can be lost if another appointment is not rescheduled within 3-6 months, or prior approval by management.

I understand that if my tattoo artist must make ANY alterations to my art, I can't decide on placement or what I like, I must pay for this time, regardless if I am getting tattooed or not.

I acknowledge that my tattoo ESTIMATE is not exact and can be more or less, depending on the actual amount of time it takes to complete my art work. I know I am responsible for the time it takes to tattoo me based on how well I slept, how much water I drank, my diet, my immune system and if I drank alcohol or smoked in the last 24 hours.

I ACKNOWLEDGE THAT THE QUOTE GIVEN TO ME IS AN ESTIMATE, AND NOT A GUARANTEE OF THE AMOUNT OF TIME IT WILL TAKE TO DO MY TATTOO, THIS INCLUDES: APPLYING MY STENCIL (IF THE ARTIST HAS TO MOVE IT, THIS IS TIME I AM PAYING FOR), PERFORMING THE ACTUAL TATTOO, AND CLEANING MY SKIN UP.

I grant Aura Creative, Inc. to use my likeness in photographs & videos in any way the company sees fit including social media and the website. By signing this release, I accept that photographs may be used on the internet or in any other print or electronic media / internet, etc. that Aura Creative, Inc. chooses. I will make no monetary or other claim against Aura Creative, Inc. for use of the photographs or video taken while I am in their studio; or any event Aura Creative, Inc. sponsors or presents.

I agree that I understand that anytime, when I book a tattoo now or in the future, if I need to reschedule or cancel my appointment I MUST let my artist know ASAP, with a minimum of 72 hours prior to my start time, otherwise my artist reserves the right to charge 50% - 100% of the time I had booked, including my deposit. I understand that by not providing sufficient notice - my artist can not make income for reserving this time for me.

I understand that anytime, when I book a tattoo now or in the future, if I need to reschedule or cancel my appointment I MUST let my artist know ASAP, with a minimum of 72 hours prior to my start time, otherwise my artist reserves the right to charge 50% - 100% of the time I had booked. I understand that by not providing sufficient notice - my artist can not make income for reserving this time for me because other clients can not arrive with short notice to fill my appointment time. I also understand that if I arrive later than 10 minutes for my appointment with or without prior notice, my appointment can be cancelled and I will be responsible for the entire time booked.

PLEASE READ CAREFULLY: - I acknowledge that I will behave in a civilized manner. I understand that I am allowed to disagree with ownership and employees, but I will express this in a polite and calm way. Should I behave irrationally, rude, condescending, etc. my artist has the right to reschedule OR CANCEL my appointment, same day, should they decide that they can not work with based on my attitude or treatment of them. If my artist cancels my appointment based on my behavior, I WILL BE 100% RESPONSIBLE for the entire appoint that day.

COVID-19 Assumption of Risk & Waiver of Liability: Aura Creative Studio puts into place preventative measures for cleaning and sanitation. We can not guarantee that you may not get sick if you choose to come in with or without a mask. I understand that I am responsible for my health and can not hold Aura Creative Studio liable for any exposure to infection that may result in personal injury, illness, permanent disability or death.

COVID TESTING - PLEASE READ - If I choose to take a COVID-19 test, I will take the test myself, no earlier than 24 hours prior, and no later than 45 minutes before my appointment. I will not let someone else take the test for me, nor will I use an old photo of a covid test, someone else's photo. If I choose to do so, my artist reserves the right to reschedule my appointment, and charge me for 100% of the scheduled time. Based on my behavior, I can also be dismissed, lose my deposit, and not be allowed to return.
