Permanent Makeup or Cosmetic Tattooing is a technique that applies permanent pigment into the epidermis that allows a desired look to be achieved that resembles makeup. It can also be used to hide scars or help with an uneven hairline. This is preformed under a sterile environment Permanent makeup uses iron oxides that pigments the skin. This pigmentation can last between 1-5 years. The pigment will gradually fade over time, however for most people it will not fade completely. 'Touch ups' of permanent makeup are advised usually after 6-8 weeks in order to achieve desired results. Continued touch- ups based on your skin's individual pigment retention, lifestyle and your desired appearance will be required every few month/years and vary from person to person, as well as in price. With every treatment there are risks involved. It is important that you understand the risks prior to undergoing treatment. Ensuring you provide a full medical history can reduce these risks but even so there may be unforeseen risks that are presented. If you have any concerns regarding these risks, do not hesitate to contact your Healthcare Professional.

Possible risks and complications from the treatment: Scabbing, Temporary pain, Scarring, Allergic reaction, Bleeding, numbness, redness, bruising, anaphylaxis, infection, and swelling.

By signing below, you are consenting to the following: 

-During the treatment, despite all the precautionary measures made by the Practitioner, injury is possible. I will not hold the Practitioner performing this service on me responsible in any issues that may arise because of having the procedure performed on me.

-I understand that there are risks associated with Permanent Makeup, if any sort of reaction occurs I will seek medical attention and inform my Practitioner.

-I understand that if I have a serious medical condition, I must come with a doctor’s note stating the condition and the clearance to proceed with cosmetic tattooing. Obsidian Brow Studio will not be held accountable for any unforeseen situations during the session.

-I acknowledge that if I must cease use of any blood thinners or OTC medications or supplements in order to receive a cosmetic tattoo, I MUST consult my doctor and provide a doctor's note. Obsidian Brow Studio will not be held responsible for any adverse medical reactions that may occur due to stopping use of medications used to treat illnesses.

-I understand that the color/outcome may not turn out as desired due to the undertone and health of my skin and that individual results will vary on each individual.

-It is my responsibility to advise the Technician of any concerns I may have before the procedure.

-I understand and agree to the aftercare instructions provided by my Technician. By not following the aftercare instructions I am aware that the desired results may not be achieved.

-I understand that once placed, my deposit is non-refundable. If I arrive with any of the contraindications or have problematic skin around the procedure area (pimples, blemishes , irritated skin, sunburn) I will forfeit my deposit because we cannot proceed with the treatment and my artist will not be able to re-fill my appointment spot  

- I understand the possible permanence of this procedure as I continue to receive touch-ups and my skin will be pigmented with iron oxides that may never fade completely.

- I understand that this service does not implant into the dermis like a traditional tattoo and that gradual fading will occur. I acknowledge that I will need to maintain touch-ups in order to avoid pigment loss.

- I acknowledge and accept there are no guarantees regarding the outcome of the procedure.

- I acknowledge that the Technician performing the procedure cannot guarantee the outcome of the procedure and will do their best to interpret my desired outcome but cannot provide an identical appearance to any reference photos provided, as all skin, facial structures, hair, and artistic styles vary. 

-I acknowledge that the Technician performing the procedure will not be held liable for and damages caused to me or my skin by any reason, including allergic reaction, skin sensitivity, and failure to follow the after care instructions.

-I have declared all relevant history, and if any changes occur in the future I will notify my Technician.

- I understand that this description of the procedure is not meant to scare or alarm me. It is simply an effort to make me better informed so that I may give or withhold my consent for this procedure.

-I acknowledge that no children or pets (except for registered service animals) are allowed in the studio.

-I understand that this is a “low” maintenance treatment not a “No” maintenance treatment. Using a pencil or powder may still be needed. Fading, blurring or poor retention can still occur.

-I understand that Touch Ups are recommended for optimal results and are charged accordingly by sessions and timeframe not by the amount of work.

-I understand the cancellation policy is a 3 days notice (72 hours) if I decide to reschedule my appointment, in order to keep my deposit. Only 1 rescheduling is allowed before a new deposit is required.

-I acknowledge that ANY same day cancellation or no-show will be charged 50% of the value of the appointment and does NOT include the originally paid deposit.

-I acknowledge that if I no-show my appointment, I will be required to pay the full value of any new appointment fee, up front before scheduling  

-I understand that there is a 15 minute grace period. Showing up beyond this grace period may result in rescheduling, forfeiting my deposit, or being charged the 50% same day cancellation fee.

-I understand that Obsidian Brow Studio reserves the right to refuse any service or fire a client at any given time.

-I consent to the permanent makeup procedure being performed on me and accept that I understand the risks and complications involved.

Pre-care instructions MUST BE STRICTLY FOLLOWED to minimize bleeding and skin sensitivities during the service: Excessive bleeding and sensitivity during the procedure can dilute and expel the pigment color, leading to suboptimal results. Your skin must be free from any irritations, including blemishes, eczema, and psoriasis. Refrain from taking Accutane for at least 12 months prior to receiving a Cosmetic Tattoo. Avoid Fish Oil, Vitamin C, Glucosamine, Evening Primrose Oil, and Ginger for one week before your appointment. These can thin your blood and hinder the pigment from implanting correctly, potentially causing your tattoo to heal patchily. Discontinue any anti-aging, skin-brightening, or anti-acne skincare products, especially those containing Alpha Hydroxy (AHA), Vitamin A, or Retinol at least 3 weeks prior to treatment. Ideally, these products should not be used for 30 days before your appointment. Even if they are not applied directly to the treatment area, they can affect underlying skin layers and may result in a patchy cosmetic tattoo. You can resume your usual skincare routine approximately 6 weeks after your final session. Do not schedule any facials, Botox, microdermabrasion, microneedling, peels, or laser treatments for a minimum of 4 weeks before your appointment and should wait at least 4 weeks after tattooing before booking again. Avoid cosmetic injections on your forehead for at least two weeks prior to your appointment. Please refrain from taking any blood-thinning medications at least 72 hours before your procedure, unless it's medically necessary. Consult your doctor before discontinuing medications such as blood thinners, Ibuprofen, Aspirin, Advil, Niacin, Voltaren, Indocid, prenatal vitamins, Vitamin E, or nutritional shakes. If you have a low pain tolerance, Tylenol is recommended. Refrain from exercising on the day of the procedure. Avoid sun tanning or tanning beds for 2 weeks prior to the procedure. If you come to your appointment with a sunburn, you will need to reschedule and will lose your deposit. As sunburned skin exfoliates, it may remove the pigment. Tanned skin is damaged skin, and your color might not heal correctly. Do not tweeze, thread, wax, tint, or undergo electrolysis for 1 week before the procedure. Scars from surgery or injuries should be fully healed for at least one year prior to the service. Avoid alcohol consumption, asprin/ibuprofin, and OTC blood thinners at least 24 hours before your procedure. UPON APPROVAL FROM YOUR DOCTOR, stop the use of prescription blood thinners, including asprin pain relief, for one week before your appointment. Refrain from consuming coffee or other caffeine products such as pre-workout and weight loss supplements for 24 hours before your procedure. Please be aware that you may experience increased sensitivity during your menstrual cycle.
