I value your pet and want to ensure you that I will make every attempt to make your pet's grooming experience as safe and enjoyable as possible. During the grooming procedure, everyone's safety comes first, for both myself and your furry loved one.

You will only need to sign it once, and it will apply to all pets you already own as well as those you may acquire in the future. Please do not hesitate to contact me if you have any questions!

Grooming can sometimes reveal a concealed medical condition or aggravate an existing one. This might happen while grooming or afterward. Please notify me of any current or previous problems so that I can keep an eye out for them. When you check the box here, the pet's owner will bear all medical fees for veterinarian care.*

Accidents
Although accidents are extremely rare when working with dogs, they do occur. Grooming equipment is sharp, and while I employ extra caution and care in all cases, cuts, nicks, scratches, quickening of nails, and other issues can occur if a dog is wiggling or moving around. The safety and comfort of your pet is my first priority. In the unlikely event that an accident occurs, you will be alerted. If I believe the situation is critical and the owner is unavailable, we will seek immediate veterinary care for your pet at the nearest veterinarian clinic.*

In the event of a medical emergency while in my care, this release grants me full authority to seek medical attention from the nearest vet clinic.*

Vaccines
It is strongly advised that all pets be up to date on all vaccinations, and Rabies immunization is required in order to get grooming services.*

Refusal of Services for Dangerous or Aggressive Animals
I reserve the right to refuse, suspend, or cancel grooming services at any time before, during, or after grooming, and the client may be charged a grooming fee if your pet is too agitated or becomes unsafe to groom (for what was done up until that point).*

Muzzles are Used
Muzzling your pet is very harmless, and it protects both the pet and the groomer. This can sometimes even calm them down, allowing the grooming process to continue. If they continue to be harmful, I reserve the right to terminate grooming services at any time. I don't muzzle unless your pet specifically needs it. Other techniques of calming your pet are explored first, and muzzling is only used as a last resort. Dogs who require muzzling maybe charged a higher fee, due to the fact that grooming aggressive or frightened dogs takes significantly longer.*

Matted Coats
Dogs with matted coats need special attention. Mats in an animal's coat can become so tight they can harm and or possibly tear the skin, provide a breeding ground for infections or infestations. By dematting, we will not cause your pet any major or unnecessary stress. Mats can be tough to remove and your per may need to be shaved. Due to skin growths trapped in the mats, removing a densely matted coat might result in nicks, cuts, or abrasions. Heavy matting can also trap moisture and urine near the pet's skin, causing mold, fungus, or bacteria to grow and cause skin irritations that were not present previous to brushing. Itching, skin redness, self-inflicted irritations or abrasions, and hair failure are all possible side effects of mat removal techniques. Pets may also experience short behavioral changes in some situations. Grooming visits should be scheduled every 4-8 weeks for the best defense. By checking the box here, you realize that your pet may need to be shaved to remove matting, and that you understand the risks involved. This method will incur an additional fee because it is time intensive and causes additional wear and tear on grooming equipment.*

Undercoat removal
Every dog sheds throughout the year, though some shed more than others. For example samoyeds, chows chows, huskys, pomeranians etc. have a double coat that requires them to be groomed every 6-8 weeks to avoid the coat from becoming impacted. Their undercoat needs to be removed by either being brushed at home daily or groomed by a professional. When the undercoat isnt removed, your dog can't regulate their own body heat, it can become quite matted and they can also get hotspots. We can only brush our dogs for so long before the skin becomes itchy or irritated. By clicking I agree you are aware that there will be an undercoat removal fee added if it is possible to remove the undercoat, if not you are aware that the appointment may have to be split into 2 for your dogs comfort.*

Cancellations/No Shows
I text or email every appointment a couple of days before to remind you of your appointment (please confirm that I have the correct info on file). If you are going to be late for your appointment for any reason, please let me know as soon as possible. All cancellations or reschedules require at least a 24-hour notice to provide me time to fill the spot.Grooming appointments will be held for 15 minutes after the stated drop-off time. Appointments that are more than 15 minutes late may be canceled. Half of the grooming fee will be charged for no-shows and appointments cancelled with less than 24 hours notice when scheduling your next appointment.*

By signing this release form, you give me permission to photograph your pet for use in customer files, as well as for the company's website and Instagram page.*

Satisfaction
Your satisfaction is extremely important to me. If you are dissatisfied with your groom for any reason, please contact me within 48 hours and I will organize a time for you to return for a free adjustment at the next available time.*

I have reviewed this Service Contract for accuracy and understand the contents of this contract. I affirm that I am the rightful legal owner of the dog(s) for which services are being rendered. I authorize this contract to be valid approval for future grooming services, permitting us to accept telephone reservations for service without additional signed contracts or written authorization.
