    • Responsibilities of the Provider
      Oppenheimer T/A Thrive Meals agrees to:

      • Review the service arrangement with the participant (or their nominee) whenever requested.
      • Deliver meals as per the participant’s order on the agreed delivery day, ensuring minimum order requirements are met.
      • Treat participants with dignity, respect their privacy, and handle personal information responsibly.
      • Actively seek feedback, address concerns promptly, and strive to improve services.
      • Maintain accurate and transparent records of services provided.

      Responsibilities of the Participant


      I, agree to:

      • Communicate openly with Oppenheimer T/A Thrive Meals to ensure the services meet my needs and promptly raise any issues or concerns.
      • Interact with Oppenheimer T/A Thrive Meals staff and delivery personnel respectfully and courteously.
      • Provide correct and up-to-date delivery and contact details.
      • If I am unavailable to accept the delivery, ensure that a safe and shaded location is specified for delivery to be left. I acknowledge full responsibility for the delivery once it is left as instructed.
      • Understand that meals are delivered with ice packs and must be stored in the fridge within 6 hours of receiving the SMS notification.
      • If living in a secure building and unable to be present, provide alternative access instructions, such as a contact person, or authorise the delivery to be left in a designated area. I acknowledge responsibility for the order once it is delivered.
      • Confirm that services will continue under my current NDIS Plan, even if the plan undergoes a review.
      • Provide at least three business days’ notice if I need to cancel a delivery.
      • Notify Oppenheimer T/A Thrive Meals immediately of any changes to my NDIS Plan, including reviews or changes to plan dates, or if I cease to be an NDIS participant.
      • Provide reasonable notice to terminate this agreement, as outlined below.
      • Recognise that ingredient costs are not covered under NDIS funding.
      • Payments can be made via debit or credit card, Apple Pay, Bank Transfer, PayPal, and Google Pay when placing an order.
      • For self-managed participants, claims can be made up to 30 days after the conclusion of their NDIS Plan.

      Termination of the Agreement
      This agreement may be ended by either party with one week’s notice. If Oppenheimer T/A Thrive Meals breaches the agreement significantly, the notice period will not apply.
