For families enrolling more than one child in our program, it's important to provide health information for each child individually. Please specify the name of each child alongside their respective health details to ensure we can accurately accommodate their unique needs and conditions.

Additionally, in the event of a medical emergency, I give full consent and authorization for any necessary medical treatment to be administered to my child(ren), including evaluation, diagnosis, treatment, and transportation to a medical facility. I understand that all reasonable efforts will be made to contact me, but if I cannot be reached, I authorize the attending medical personnel to provide the care deemed necessary for the health and safety of my child(ren).
