Required for all Susan Peavey Travel bookings
I. BOOKING AUTHORIZATION FOR SUSAN PEAVEY TRAVEL
To complete your booking with Susan Peavey Travel and confirm your arrangements, your completion of this form including Credit Card payment authorization, is required in full. This form includes three steps including your acceptance/denial of travel insurance, your acceptance of our terms & conditions, and your authorization to charge your credit card for payment of deposit of final payment.

Once finished, simply click SUBMIT and the form will be submitted to the Susan Peavey Travel team fror review. Credit card charges will not be processed automatically - this information will be processed manually by the Susan Peavey Travel team through the appropriate travel supplier(s) used for your travel booking.

NOTE: Once you submit this form, we will contact you for your credit card information. Final travel documents cannot be sent to you until this form is submitted. Please contact us with any questions you may have.
