    This form is to gather information on how to best serve athletes mental side of performance. I will host four mental training sessions throughout the year where participants who fill this out will have the opportunity to attend for free. You will also be enrolled in a monthly mental training email where I will write about more detailed concepts and practices to help performance and free our minds from the natural anxiety associated with pressure. Everything will be kept confidential and I intentionally left out a name spot so players or parents can remain anonymous.
