I am aware of the risk of attending Type 1 Unbound events. I promise not to hold Type 1 Unbound its officers, agents, or employees liable or pursue legal action against them. This waiver absolves Type 1 Unbound of any duty for injuries sustained to myself or any minors accompanying me and under my supervision on the premises before, during, or after the activity.
I understand that participation in the Type 1 Unbound event involves inherent risks, including the possibility of injury, illness, or loss. By signing this agreement, I voluntarily choose to participate and assume responsibility for myself and all minor children under my care and supervision.
In consideration of being permitted to attend and participate in the event I agree to the following:
1. Release of Liability
I will not hold Type 1 Unbound, its leaders, volunteers, employees, or representatives liable for injuries, damages, losses, or expenses arising from participation in event activities, except as prohibited by law.
2. Acknowlegement of Unattended Minor
I agree that the minors who are not directly supervised by myself of a caregiver are responsible for their own conduct and their own personal health management, all diabetes needs, treatment, medication is self adminisitered and Type 1 Unbound is not responsible for medical care.
