As the legal owner of the pet(s), I voluntarily request courtesy-posting assistance from Street Paws, Inc. and permission to attend approved adoption events with my pet(s). I agree to comply with all applicable rules, policies, and instructions established by Street Paws, Inc. Failure to comply may result in removal of my courtesy posting and/or immediate termination of my event participation.I agree to conduct myself respectfully and professionally and to provide accurate and complete information about my pet(s), including known medical conditions, behavioral concerns, and any history of biting or aggression. I understand that my participation does not make me an employee, volunteer, agent, or authorized representative of Street Paws, Inc.Unless otherwise agreed in writing, I remain responsible for the care, supervision, handling, and control of my pet(s), including during events. My participation is voluntary and without pay or compensation.

Pet Owner’s Ability to Participate Safely: I certify that I am able to safely participate in approved adoption events with Street Paws, Inc. and to safely supervise, handle, and control my pet(s). I agree to follow all safety instructions and to participate only in activities I can safely perform. I will promptly inform a Street Paws representative if any condition, limitation, or circumstance affects my ability to participate safely or manage my pet(s). I acknowledge that I am responsible for managing my own health and well-being during my participation and will request assistance or stop participating if I cannot do so safely.

I am not aware of any condition or limitation that would prevent me from safely participating in approved adoption events or safely supervising, handling, and controlling my pet(s).

Assumption of Risk: I understand that attending adoption events with my pet(s), handling animals, and being near other animals carries inherent risks. These risks may include, but are not limited to, bites, scratches, kicks, animal attacks, falls, allergic reactions, exposure to unvaccinated animals, and zoonotic diseases (diseases that can be transmitted from animals to humans). I also understand that animals may behave unpredictably, escape, or injure people or other animals, and that my pet(s) may be exposed to illness or injury.I voluntarily participate with an understanding of these risks. I agree to follow all safety instructions and use appropriate leashes, carriers, or other required restraints to maintain control of my pet(s). If I am injured, I will seek necessary medical attention and remain responsible for my medical expenses, subject to any applicable insurance coverage. If I am pregnant, become pregnant, or have a condition that compromises my immune system, I should consult my healthcare provider about appropriate precautions before participating in activities involving animals. I understand and voluntarily accept the inherent risks associated with my participation.

Transportation: I understand that transporting my pet(s) to and from adoption events involves risks, including vehicle accidents, personal injury, property damage, and injury to or escape of my pet(s). I am responsible for arranging safe transportation and appropriately securing my pet(s) during travel. When using my personal vehicle, I am responsible for maintaining legally required automobile insurance and complying with applicable traffic laws. I understand that courtesy-post participation does not itself provide automobile insurance coverage through Street Paws, Inc.

Use of Event Equipment: I understand that participating in adoption events may involve using carriers, crates, cages, kennels, gates, leashes, and other animal-handling or event equipment. I agree to use equipment only as authorized, follow all safety instructions, and promptly report damaged equipment or unsafe conditions to a Street Paws representative. Before placing my pet(s) in any enclosure, I will check that it is appropriate for my pet(s) and securely latched. I understand that improper use or equipment failure may result in injury or animal escape, and I voluntarily accept the inherent risks associated with using this equipment.

Insurance: I understand that participating as a pet owner in courtesy posting or adoption events does not make me an employee or registered volunteer of Street Paws, Inc. I will not assume that Street Paws’ workers’ compensation or volunteer accident insurance covers my participation. Any available coverage is determined by the applicable insurance policy’s terms, eligibility requirements, limits, and exclusions.

Media Consent: I authorize Street Paws, Inc. to use my name and photographs, video, or audio recordings of me and my pet(s) that I provide or that are taken during my participation in approved events. These materials may be used for courtesy posting, adoption promotion, education, advocacy, fundraising, and other rescue-related communications, including websites, social media, emails, news coverage, advertisements, and printed materials.I confirm that I own or have permission to authorize the use of any materials I submit. I grant Street Paws, Inc. permission to reproduce, edit, and publish these materials for the purposes described above, without additional notice, approval, or compensation. This permission does not transfer ownership of materials I submit. To the extent permitted by law, I waive privacy and publicity claims arising from the authorized uses described in this consent.

Ownership and Financial Responsibility: I certify that I am the legal owner of the pet(s) submitted for courtesy posting and have authority to make decisions regarding their care and placement. I understand that courtesy posting and participation in adoption events do not transfer ownership or custody of my pet(s) to Street Paws, Inc.

Unless otherwise agreed in writing, I remain responsible for my pet(s)’ care and all related expenses, including veterinary care, medical treatment, food, transportation, and boarding. Street Paws, Inc. has no obligation to pay these expenses or accept my pet(s) into its care.

Any adoption, foster placement, or transfer of ownership must be approved by me as the legal owner unless otherwise agreed in writing. I agree to promptly notify Street Paws, Inc. if my pet(s) are placed, transferred, or no longer available so that courtesy postings can be updated or removed.

If Street Paws, Inc. agrees to pay expenses for my pet(s), any reimbursement obligation will be documented in a separate written agreement identifying the covered expenses and circumstances requiring repayment, including any applicable placement or transfer to another person, rescue organization, or animal control agency.

Release of Liability: In consideration for receiving courtesy-posting assistance and being permitted to participate in approved adoption events with my pet(s), I knowingly and voluntarily assume the risks, whether known or unknown, associated with my participation, including attending events, handling animals, using event equipment, and transporting my pet(s) to and from events.To the fullest extent permitted by Georgia law, I release and discharge Street Paws, Inc., its officers, directors, employees, agents, volunteers, contractors, sponsors, and participating event hosts and property owners, including Hollywood Feed, PetSmart, and PetSmart Charities when involved in the applicable event (collectively, the “Released Parties”), from claims arising out of my participation for personal injury, illness, death, property damage, or injury to, illness of, loss of, or death of my pet(s), including claims caused by the ordinary negligence of a Released Party.I understand that this agreement waives substantial legal rights, including rights to seek compensation for claims covered by this release. I further agree not to bring a civil lawsuit against a Released Party for a claim covered by this release.This release and agreement not to sue do not apply to gross negligence, reckless conduct, willful or wanton misconduct, intentional harm, or any liability that cannot lawfully be waived. Nothing in this agreement prevents me from reporting an incident to law enforcement or a regulatory agency.To the extent permitted by law, this agreement is binding upon me and my heirs, executors, administrators, successors, and assigns.

The release and agreement not to sue apply to claims they may bring on my behalf or through me, subject to the same limitations and exceptions stated in the Release of Liability. This provision does not waive any independent rights or claims belonging to another person.

I understand that this Waiver and Release of Liability benefits Street Paws, Inc. and the other Released Parties identified in the Release of Liability section. My release of claims and agreement not to sue apply equally to those Released Parties, subject to the same scope, limitations, and exceptions stated in that section, including the exceptions for gross negligence, reckless conduct, willful or wanton misconduct, intentional harm, and liability that cannot lawfully be waived.

I knowingly and voluntarily agree to the waiver of rights described in this Waiver and Release of Liability, subject to its stated limitations and exceptions. I understand that Street Paws, Inc.’s agreement to provide courtesy-posting assistance and permit my participation in approved adoption events constitutes consideration for this agreement. I understand that this assistance does not guarantee adoption or placement of my pet(s).

I HAVE CAREFULLY READ THIS WAIVER AND RELEASE OF LIABILITY, UNDERSTAND ITS TERMS, AND SIGN IT KNOWINGLY AND VOLUNTARILY. I UNDERSTAND THAT I AM GIVING UP CERTAIN LEGAL RIGHTS, SUBJECT TO THE LIMITATIONS AND EXCEPTIONS STATED IN THIS AGREEMENT. I CERTIFY THAT I AM AT LEAST EIGHTEEN (18) YEARS OLD AND AM THE LEGAL OWNER OF THE PET(S) IDENTIFIED IN THIS FORM.

Severability:
If any provision of this agreement is determined by a court of competent jurisdiction to be invalid or unenforceable, the remaining provisions shall remain in effect to the extent permitted by law.

Governing Law and Venue:
This agreement shall be governed by and interpreted under the laws of the State of Georgia. To the extent permitted by law, any lawsuit arising out of this agreement shall be brought in a court of competent jurisdiction in Henry County, Georgia, or, if federal jurisdiction applies, in the federal court serving Henry County, Georgia.
