To courtesy post my pet(s), I have voluntarily applied with Street Paws, Inc., and/or its affiliates and in doing so, I agree to comply with all rules and regulations established by Street Paws, Inc. and I understand that failure to do so may result in my immediate termination as a volunteer and elimination from our courtesy program. As a volunteer, I agree to do my best to represent Street Paws, Inc. to the public in an accurate and professional manner. I acknowledge that my services are provided strictly on a volunteer basis, without pay or compensation of any kind, and without liability of any nature on behalf of Street Paws, Inc. and all services are to be performed at my own risk.

I recognize that in handling animals and performing other tasks, there in exists a risk of injury including physical harm caused by the animals. I understand if an accident or injury should occur, no matter how minor, that I will seek any necessary medical attention utilizing my own medical insurance. I understand that if I am or become pregnant or have any medical conditions that compromise my immune system while volunteering for Street Paws, Inc. that I should talk to my doctor and get my doctor’s approval to volunteer with rescue animals.

I understand and acknowledge that Street Paws, Inc. has voluntarily offered me the opportunity to have personal contact with one or more animals within its programs. I understand and acknowledge that I have voluntarily selected the animal(s) I wish to have personal contact with and I knowingly and freely assume the risk of such contact, both known and unknown, to include the following but not limited to any and all bites, scratches, violent attacks, other personal injury and/or property damage caused by my exposure to and/or handling of such animal(s). In exchange for the opportunity to visit/interact with said animal(s), Street Paws, Inc. and its volunteers, representatives, board members, officers, members, promoters, sponsors, advertisers, owners and lessees are hereby released, waived, discharged from any and all liability for any and all injuries, loss, any claim, damages and/or death resulting from such exposure and contact and covenants not to sue Street Paws, Inc and others hereby specified in this waiver. Releasor waives any and all claims against Petsupermarket, Petsmart, and Petsmart Charities. By performing this volunteer work, I agree as follows and have initialed each item to indicate that agreement:

I am aware that this is a contract between me and Street Paws, Inc. and that it waives legal rights that I may have now or in the future and releases Street Paws, Inc. and other from claims for damages.

I am voluntarily participating in this activity with full knowledge of the tasks involved. I agree to accept any and all risks associated with my participation, including but not limited to injury and illness.

I understand there are risks and dangers associated with working with wild, feral and domesticated animals, unvaccinated animals, including but not limited to bites, scratches, zoonotic diseases (diseases transmitted from animals to humans), and allergic reactions. I also understand there may be risks involved with exposure to certain chemical cleaning products while performing my volunteer duties. I fully understand and accept those risks and dangers.

I fully assume all the risks involved with my volunteer activities and acknowledge that they are acceptable to me. I agree to use my best judgement in undertaking these activities. I also agree to follow the rules and safety instructions as given to me by Street Paws, Inc. employees and volunteers as authorized to act in a supervisory capacity.

I agree that I will not sue, prosecute, or in any way make a claim against Street Paws, Inc. for injury to me or damage to my property resulting from the negligence or other acts, howsoever caused, by any employee, agent, volunteer or contractor of Street Paws, Inc. or other people as a result of my duties.

I/Releasor, being of lawful age, in consideration of being permitted to volunteer with Street Paws, Inc., does for myself, my heirs, executors, administrators, and signs, releases and hold harmless, and discharges Street Paws Inc., its officers, members, volunteers, promoters, sponsors, advertisers, owners and lessees of the premises and each of them, their officers and employees, their heirs, administrators, and executors of and from any and every claim, demand, action or right of action , of any kind or nature, either in law or in equity arising from or by reason of any bodily injury or personal injuries known or unknown, death and/or property damage resulting or to result from any accident which may occur as a result of volunteering or working with the animals cared for by Street Paws, Inc. I fully and forever release and discharge Street Paws, Inc. from any and all actions causes of action, claims, liabilities, or demands I have or may have in the future, whether known or unknown, for injury, death or damage arising out of or related in any way to my volunteer duties whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.

I agree that Street Paws, Inc. may use my name, and pictures, photographs, or video and/or sound recordings of me on television, on radio, on the internet, in emails, social media, in stories, news articles, advertisements, or other written or digital materials. I agree that such uses may include education, advocacy, and fundraising. I consent to and authorize, in advance, such use and agree that Street Paws, Inc. does not have to notify me of such use or provide me with other consideration for such use. I waive any rights of privacy and/or publicity I may have in connection with these uses and I hereby release any such photographs to Street Paws, Inc. for use in its programs, publications, social media, and other purposes.

I agree that the rights I am giving up and agreements I am making apply equally to me and to my heirs, successors, assigns, guardians and legal representatives. I agree that none of those individuals may make any claim or take any action that I could not make or take myself.

I agree that this Waiver and Release of Liability protects and is for the benefit of Street Paws, Inc., and for its affiliates, and their respective employees, officers, directors, consultants, interns, volunteers, licensees, and all others acting on their behalf. I also agree that I may not make any claim or take any action against any of those affiliates or individuals that I could not make or take against Street Paws, Inc. itself.

To the extent that legal consideration is required for this Waiver and Release of Liability to be effective, I agree that I have received good, valuable and sufficient consideration by being permitted by Street Paws, Inc. to provide volunteer service and to receive training and instruction.
