  • *Please note: A medication administration consent form is required to be on file for any student that stores prescription medications at Woodside Nursery School (EPI pen, inhalers). Medications will only be administered for life threatening situations such as asthma attack or allergic reaction. Please inquire with your child's pediatrician about obtaining the proper paperwork.

  • I have read, understand, and agree to the Woodside Nursery Parent Manual.*
  • Every family at Woodside Nursery School has an annual fundraising goal to meet to help offset the costs of program operations and supplies. This rate is determined annually by our executive board based on our budget. The fundraising requirement is $200.00 per child enrolled. We set up the fundraisers throughout the year or you can opt to buy out this fee.*
  • Our executive board plans family events throughout the year for our families to connect, socialize and build relationships. We ask that every family volunteers to help out at 2 events per year. We will provide a sign up with dates. Duties will be determined based on response per event. You can choose to opt out of events by paying a fee of $50.00.*

  • I understand that my registration and enrollment is not considered complete until I pay the non-refundable $100.00 registration fee. I understand that registration and enrollment is accepted on a first come, first serve basis for all programs. We will email you within 10 days of completion of this form. If registration is accepted, this email will include your total due and payment breakdown. I understand that tuition installments are due beginning 4/1/2025.*

  • I understand that I must submit my child's heath appraisal form no later than 8/1/25 for the 2025-2026 school year. This form must be obtained by your child's pediatrician and can be submitted digitally or in a hard copy.*
  • I understand that completion of this form and my signature below are my official contract. I have read and understand the parent manual. I agree to pay all fees in full based on the selections I have made. I understand that any amendments to this agreement must be communicated in writing to the school coordinator and approved by the Woodside Board of Directors. I understand that if I do not fulfill my duties as agreed upon in this contract, I will be responsible to pay back the discounts applied.*
