Passenger Information Form
To complete a booking, the following information is required from all travelers:

1. Government-issued travel documents you intend to use on this vacation must match the information you provide on this form.
2. No nicknames or shortened versions of names will be allowed.
3. If erroneous information is provided, it may result in denial of travel or additional fees.
IMPORTANT:

Please use name as it appears on your U.S. passport or ID.

(The birth certificate cannot be the short form and cannot be an abstract)

All names must match whichever form of identification you plan to use.

Please note that there are additional requirement for minors traveling without both parents.
