This information should be brought to your appointment or uploaded securely through your client portal, let me know if you need a link to upload).

  • At any time during 2025, did you receive (as a reward, award or payment) or sell, exchange, give or otherwise dispose of a digital asset (or any financial interest in a digital asset) examples: bitcoin, NFT's, etc.?*
  • At any time last year, did you receive funds for personal or business purposes through a third party payment processor (Paypal, Venmo, Square, Cash App, or any similar payment processing platform)?*
  • Did you or anyone in your household have coverage through the healthcare marketplace?  (You must have form 1095-A from the marketplace login for this if yes you can login at www.healthcare.gov)

    General questions that apply to all credits on tax returns.
  • Have you ever had a credit disallowed or reduced in a prior year? (If you haven't received an IRS communication about this, you can select "No" below.)
  • Were you (and your spouse, if applicable) a US citizen, US National or resident alien all year?
  • Did you (and your spouse if applicable) live in the US all year?
  • Did you (or your spouse, if applicable) have income from outside the US?
  • Claiming Dependents/Head of Household Questions

    Determines which types of dependent credits can be claimed on your returns.
  • Are/were you separated from your spouse (if no spouse please mark "no" below)?
  • If "yes", did you live with your spouse at any time during the last six months of the year?
  • Did your child(ren), stepchild(ren) or foster child(ren) live with you during the tax year?
  • Do you have school, daycare or medical records for each child as evidence of attendance from your address? These documents must show the parent's name, child's name and address. IRS MAY REQUEST COPIES OF THESE DOCUMENTS. WITHOUT THESE DOCUMENTS YOU AND PREPARER MAY BE SUBJECT TO PENALTY. Please provide a copy to your preparer.
  • Is any child listed on your return married and filing a joint tax return with their spouse?
  • Did you (and spouse, if applicable) pay more than half of the cost of maintaining your home for you and your qualifying dependent?
  • Is your (and your spouse, if applicable) income sufficient to support yourself and your family?
  • Did any dependent on your tax return make more than $5,050 during the tax year?
  • Did anyone living with you (aside from a spouse) help pay living expenses including toiletries, groceries, clothing, rent, utilities, insurance, property taxes?
  • Did anyone in your household (aside from your spouse) have more income than you?
  • Did anyone not listed on your tax return live with you during the tax year?
  • Did any adult (other than your spouse when applicable) live in the same household as the dependent or child(rent) during the tax year?
  • Did anyone on your tax return live with you for less than six months?
  • Higher Education Credits

    Determines eligibility for Education credits.
  • If someone attended college during the year, did they receive a 1098T? (a copy of the tax form 1098T is required and can usually be printed off the student's school portal under "tax forms").
  • Were they pursuing a degree or credential?
  • Were they enrolled at least half time?
  • Did they have a bachelor's degree before this tax year?
  • Have they claimed the American Opportunity or Hope tax credits in four previous tax years?
  • Have they been convicted of a drug felony?
  • Do you have receipts or financial records for qualifying expenses? (Not required for tax preparation, but you should keep a copy with your files in case the IRS requests them in the future).
