Media Waiver and Release

I, {parentguardians} give my consent for Soleil Music and Arts to photograph and/or video and audio record my child(ren) and/or me or use photograph(s) or videos that already exist of my child(ren) and/or me that were taken while participating in or attending a Soleil Music and Arts event. I understand that the photographs, digital images, or video segments may be used in print or electronic media and that the photographs may be displayed on websites owned or sponsored by Soleil Music and Arts. I,{parentguardians} give my permission to publish, display, and distribute these materials. Possible uses include educational, future event promotions, marketing, or other legal purposes. I understand that Soleil Music And Arts owns the copyrights to the multimedia material in which I, and or my child may appear.

I, {parentguardians} understand, that by enrolling in this program, Soleil Music and Arts may also use any written evaluations, comments, or letters written by myself or by my child in its promotions. These photographs/videos/audios/written material will remain the property of Soleil Music and Arts and may be used in advertising or marketing campaigns on Soleil Music and Arts and/or partner websites and for promotional and informational material including, but not limited to: flyers, brochures, newsletters, emails, advertisements, newspaper articles, TV or cable interviews/promotions, Facebook, Twitter, Instagram, and YouTube. Students are not identified by their full name; if there is a desire by Soleil Music and Arts to identify students in materials using their full name, the parent/guardians of that student will be contacted for permission. I, {parentguardians} hereby waive and release on behalf of myself and/or my child, any rights to compensation for or ownership of such images written material and/or sounds.

Soleil Music and Arts General Liability Release:

I, {parentguardians} do hereby waive, release, absolve, forever discharge, and do further agree to indemnify and hold Soleil Music and Arts, its directors, employees, trustees, officers, volunteers, and agents harmless from and all claims, damages, losses, and/or expenses arising out of my child’s participation in Soleil Music and Arts activities. I assume all liability for any and all personal injury, bodily injury, illness, or property damage that myself, my spouse, or my child(ren) might incur as a result of participation in such activities. I also agree that we will not bring any lawsuits nor make any demands nor pursue any complaints against Soleil Music and Arts as a result of their participation in Soleil Music and Arts activities. Agreement to this Release also warrants that participation in these activities are voluntary and that the participant(s) and the undersigned understand the inherent risks involved. The participant understands that these risks exist despite the safety precautions and procedures implemented by Soleil Music and Arts. The participant agrees to obey all rules and policies mandated by Soleil Music and Arts personnel and included in the Soleil Music and Arts Code of Conduct.

In consideration of Soleil Music and Arts agreement to allow my child(ren) to participate in their performance events, I, {parentguardians} accept full responsibility for any injury or accident that may occur to myself, my spouse, or my child(ren) while participating with Soleil Music and Arts. I give permission for my child(ren) to participate in performances that occur on the grounds of the selected venues of each performance. These activities may include, but are not limited to, performing on stage, pre-performance rehearsals, or in a waiting area supervised by an employee, intern, parent volunteer, parent or guardian. I, {parentguardians}, the parent or guardian agree to accompany my child(ren) at all times at performance venues. I, {parentguardians} also agree that, at no time will I leave my child(ren) unattended at the venue. I also understand that, although Soleil Music and Arts has taken reasonable steps to select venues for performances that are generally considered safe, there is no expectation that there will be security guards, cameras, or law enforcement officers present. I, {parentguardians} will take precautions to familiarize myself and my family with the location of emergency exits, secure their property, including musical instruments, and other items. I agree that Soleil Music and Arts cannot be held liable for any damaged or stolen property.

Confirmation

BY ACKNOWLEDGING AND SIGNING AND/OR TYPING MY NAME BELOW, I, {parentguardians} AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

Soleil Music and Arts Chamber Group participants are asked to commit for the remainder of the 2024-25 school year. Students need their parent's support to be successful.

1- Participants are expected to be present at each rehearsal. If there is a need to miss a rehearsal due to personal reasons or other school/life commitments, the director needs to be notified as soon as possible.

2- Each student accepts the responsibility of rehearsing the selected music before each meeting and arriving to each rehearsal prepared to play the best they can.

3 - Rehearsal Time: Rehearsal time will be spent learning new or improving skills, reviewing, discussing, and playing musical selections, Short breaks will be taken, and student will enjoy snacks if we have them.

Events:

As a Chamber Group member, Students are required to participate in all scheduled events. Participation is mandatory and vital to the success of the events, as a Chamber group, by design is small, and may only contain one or two members per instrument part.

If for some reason your child will be unavailable on any of the event days, please let us know as soon as possible. Unexcused absence from scheduled events & the final rehearsal before an event may result in the student's expulsion from the group program. Our goal isn't to punish student & parents, we aim to instill a sense accountability and teamwork in our youth.
