Company, or any of its employees or agents, from any and all claims and/or liability arising out of, connected with, or in any way associated with the program and/or my participation in the program.

MEDIA RELEASE FORM : The undersigned does hereby authorize Pink Eyebrows, LLC. to take, or to have taken, still photographs and/ or videos of participant and consents and authorizes Pink Eyebrows, LLC. to use and reproduce the photographs and/or videos.
