Program Details

Our Mini Shamrock Basketball program has been a staple here at St. Patrick Catholic School thanks to our passionate volunteer coaches led by Program Director Sarah Strickler. Each week players will work on skills for approximately 30 minutes followed by 30 minutes of games and fun contests.

Medical Alerts and Waiver of Liability and Authorization for Emergency Medical Care

I/We hereby authorize the principal, or his/her designee, athletic director, coach or sponsor in attendance at any St. Patrick Athletics contest or event to select, secure and consent to necessary medical attention for my child resulting from injury, illness or accident requiring medical care while I/We are not in attendance. I/We hereby release St. Patrick Parish and School and such person from any and all liability on account of such selection or authorization and for any and all damages which may occur on account thereof.

Parent(s) or legal guardian of above student must consent to Waiver of Liability and Authorization for Emergency Medical Care in order for his/her child to participate in St. Patrick Athletic Program.

Assumption of Risk

In consideration of my child being allowed to participate in this sport, I hereby agree on behalf of myself and my child, to release St Patrick School and/or Parish, the Roman Catholic (Arch)diocese of Detroit and Lansing, and any and all affiliated organizations, their employees, agents and representatives (Releasees) from any and all claims, including negligence, which may be asserted by me or my child, or on behalf of my child, arising from or relating to my child’s participation in the sport.

In the event this release on behalf of myself and/or my child is held to be invalid or unenforceable, I hereby agree to indemnify and hold harmless Releasees from any and all claims, including negligence, which may be asserted by me or my child, or on behalf of my child, arising from or relating to my child’s participation in the sport. This release of indemnification does not apply to claims for intentional misconduct or gross negligence; nor does this release or indemnification apply to the extent of commercial insurance coverage for any claim, but this Release or Indemnification shall apply to the extent of any self-insurance or deductible applicable to any claim.

Sports Physical

During this time, physical safety is of the utmost importance. All athletes must have a sports physical dated after April 15, 2024. The physical form must be turned in no later than the first practice in order for the athlete to participate.

If your doctor indicates that your child is able to participate in sports, you may use your annual physical or the Sports Physical Form below.

The Sports Physical form can be mailed to St. Patrick School, 1001 Charles Orndorf Drive, Brighton, MI 48116 via U.S. Mail or electronically (athletics1@stpatschool.org).

*Athletes will NOT be allowed to participate without a physical or documentation from your pediatrician indicating your child may participate in sports.*
