Overview:
Hart to Hart Home Care, LC is looking to hire a Registered Nurse to join our growing team. Whether recovering from illness, injury or surgery, living with a chronic disability or dealing with the natural process of aging, our services are tailored to meet the individual needs of people of all ages, physical conditions and cognitive abilities. The RN will meet with new clients to create care plans (Home Care Assessments) and follow up with current clients to ensure quality of services. Great for nurses with a clinical background who enjoy nurse education and visiting clients in their homes!
Our clients reside in the following communities: Clayton, Dekalb, Henry and Spalding counties.
Responsibilities:
- Completes an initial assessment of client and family to determine home care needs, making necessary revisions per physician orders as status and needs change.
- Completes client home supervisory visits to give an update of care every 60-90 days.
- Effectively communicates, builds personal rapport and establishes trust with clients, family members, and caregivers.
- Provides education to clients and family members on resources, self-care techniques, and/or medical conditions per physician orders.
- Understands and attempts to meet the needs, wants, and expectations of clients.
- Listens and responds to calls for help from other direct care staff when needed.
- Annually assess the performance of Caregivers, PCAs, and CNAs during a (3) hour stay with the clients and staff, and will assess their work performance.
- Will ensure Caregivers, PCAs, and CNAs complete all tasks and provide competent services as indicated in the client’s care plan. All observations will be noted in the caregiver's file, and observations will be noted in the caregivers form.
- The RN will evaluate the quality and results of care being rendered. During this visit the supervisor will evaluate the client's health status noting changes in medical condition, medications etc. Any revision will be noted in the clients file form.
Requirements:
- RN with current licensure to practice professional nursing in the State of Georgia.
- RN must have 5 years of nursing experience. Home health/hospice preferred.
- Must possess and maintain current CPR and First Aid certification.
- Meets the requirements for employees providing direct client care.
- Valid driver’s license, auto insurance and reliable transportation.
