Hello! Your time filling out this form is greatly appreciated and I look forward to hearing all of your ideas. My policies are listed below for your review, please make sure you have looked over these policies thoroughly before submitting this form.

Policies:

*Incantation Tattoo is a safe space for myself and clients. Here we accept and embrace eachother, anything less will not be tolerated.

*Deposits of $100 are required to book appointments. Payments for deposits can be accepted in person with cash, and through Venmo or Cash App. Deposits are nonrefundable and deducated from your sessions total.

*Deposits are transferable to another date with 24 hour prior notice to the appointment. If 24 hour notice is not given your deposit wil not be transfered and another will be required to reschedule.

*Please reschedule if you are feeling under the weather. Your honesty is appreciated and you will happily be rescheduled.

*I do not tattoo anyone below the age of 18. Please respect my preference, there are no acceptions.

*Designs will not be sent out prior to our appointment. All designs will be reviewed and updated together during your appointment. This is the best way for me to recieve your notes with the most efficiency.

*Please make sure to arrive no earlier than 10 minutes to your appoinment.

*The studio is cash only for appoinments. Make sure you are prepared.
