PARK RIDGE HAWS LIABILITY WAIVER

I recognize and understand that youth football is a physical contact activity and that my child's participation might result in serious injury, including permanent disability or death. I recognize and understand that such risk may be due to not only my own child's actions, but also the action, inaction or negligence of others. I recognize that there may be other risks that are not known to me or to others or not reasonably foreseeable at this time. I assume all of the foregoing risks and I accept personal responsibility for any damages that may result from injury, permanent disability or death. I enroll sign my child up for youth football entirely of my own free will and understand the importance of following the rules of training and competition. I certify that my child is in good physical condition, and has no disease, injury or other condition that would impair performance or physical and mental well-being during training practice and/or competition. If my child has any injuries or illnesses that could affect ability to participate, I will notify the coaches and discuss the appropriateness of youth football with my or my child's doctor. I grant permission in case of injury to have a doctor, nurse, athletic training or other emergency medical personnel provide my child with medical assistance or treatment for such injury. The cost of any treatment or hospitalization arising from injury during training or play shall not under any circumstance be paid by Park Ridge Hawks Football, its affiliated organizations and governing bodies, their officers, coaches or other members of the organizations, participants, supervisors, coaches, sponsoring organizations or their agents. I release, waive, discharge and covenant not to sue, Park Ridge Hawks Football, its affiliated organizations and governing bodies, their officers, instructors and coaches, other members of the organizations, participants, supervisors, coaches, sponsoring organizations or their agents, and if applicable, owners and leasers of the premises from any and all liability to the undersigned, his or her heirs and next of kin for any and all claims, demands, losses and damages which may be sustained and suffered on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasees or otherwise.

MEDIA RELEASE
I understand that photos and videos from Park Ridge Hawks Football games, practices and events may be used in promotional material for the team. I agree to allow my image or my child's image to be used for this purpose and that I will not be compensated. I also understand that once a promotional item has been produced, it is not possible to remove my image or my child's image from the production.

ROSEMONT WAIVER

Please read this form carefully and be aware that, in signing up and participating in the above program/activity, you will be waiving and releasing all claims for injuries, arising out of this program/activity that you or the above participant(s) might sustain. The terms “I,” “me,” and “my” also refer to parents or guardians as well as the participants in the program/activity. In registering for the program/activity, you are agreeing as follows:
As a participant in the program/activity, I recognize and acknowledge that there are certain risks of physical injury and I agree to assume the full risks of any injuries, including death, damages or loss which I may sustain as a result of participating, in any manner, in any and all activities connected with or associated with such program/activity. I further recognize and acknowledge that all athletic activities involving strenuous exertion or potential body contact are hazardous recreational activities and involve substantial risks of injury. I agree to waive and relinquish any and all claims I may have as a result of participating in the program/activity against the Rosemont Park District, any and all other participating or cooperating governmental units, any and all independent contractors, officers, agents, servants, volunteers and employees of the governmental bodies and independent contractors and any and all other persons and entities, of whatever nature, that might be directly or indirectly liable for any injuries that I might sustain while participating in the program/activity. (The parties described in the preceding sentence are referred to as “released parties” in the remainder of the Agreement.) I do hereby fully release and discharge the Rosemont Park District and any and all other released parties, from any and all claims resulting from injuries, including death, damages and losses sustained by anyone and arising out of, connected with or in any way associated with my conduct and the activities of the program/activity. I further agree to indemnify, hold harmless and defend the Rosemont Park District and any and all other released parties, from any and all claims resulting from injuries, including death, damages and loss sustained by anyone and arising out of, connected with, or in any way associated with my conduct and the activities of this program/activity. I further understand and agree that the terms such as “participation,” “program” and “activities,” referred to in this Agreement, include all exercises and physical movements of any nature while I am participating in the program/activity and further include the provision of or failure to provide proper instructions or supervision, the use and adjustment of any and all machinery, equipment and apparatus and anything related to my use of the services, facilities or premises involved in the program/activity. I understand the nature of the program/activity for which I am registering and have read, and fully understand this Waiver, Release and Hold Harmless Agreement. I further understand that any advisements or warnings of the particular risks of this program/activity that I subsequently receive will be incorporated by reference into and become a part of this Agreement.
I have read and fully understand the above risk warnings of the program/activity. I understand that the Rosemont Park District does not provide insurance or protection against injuries sustained by program/activity participants and I fully accept the risk of injury. I also understand and agree that this document is valid in and of itself as a waiver and release form and adds to or confirms any promises made in a Waiver, Release and Hold Harmless Agreement included in the registration for this program/activity.
