  • Dear Parent(s) and/or Guardian(s),

    May 07 - 10, 2025 students will travel to various colleges, universities, and other sites throughout the State of Maryland, Virginia, and in the District of Columbia. 

    Your child will not be able to attend this trip without consent and permission through this online form. School rules regarding conduct and behavior are in effect during the duration of the entire trip. Any student who violates enforced school rules and Trip Guidelines will be eliminated from future outings (please consult your district’s rule book for further details).

    By continuing with this form, you give permission for your student to attend the Detroit Academy of Arts and Science's HBCU Tour powered by The Motivational Foundation on May 7 - May 10, 2025. You understand that you are responsible for understanding all trip guidelines, payment deadlines (if any), and will not be refunded if your student is unable to attend the trip. You understand that the trip is a learning experience and ANY inappropriate behavior from your student, as deemed unacceptable by the School Staff and The Motivational Foundation's staff, and chaperones for this trip, could result in your child’s immediate dismissal and suspension from the duration of the trip at your expense.

  • I understand that the Motivational Foundation is not responsible for any of the following including, but not limited to: student sexual misconduct, injury, illness, or fatality. I also understand if my student conducts him/herself in a way in which legal action must be taken, that I am fully responsible for my student's actions. Any student that violates enforced code of conduct and trip guidelines will be eliminated from future outings, and may be returned home before the conclusion of the trip, at the parent's/guardian's expense. (Please consult your school district's Code of Conduct handbook for code of conduct compliance). I understand that anything my child brings on this trip is at our own discretion, own security, and own responsibility. However, I further understand that The Motivational Foundation recommends that my child does not bring any items of large value. However, if my student chooses to do so, The Motivational Foundation WILL NOT assume responsibility for broken, stolen, and/or missing items.

    Media Consent: I give my student permission to be photographed on this trip and I also allow The Motivational Foundation access to those photos to promote this College Tour initiative with my child’s image on The Foundation’s website, affiliate sites, and other paraphernalia. I waive the right to sue The Motivational Foundation from that which is mentioned above, and I am in compliance with this liability contract.

    I understand I am responsible for reading and adhering to the following information. By signing this document and by uploading a copy of my (Parent/Guardian) State Issued Identification, I give my student permission to attend, and I (both parent & student) am in compliance with this binding agreement and waiver of liability.
