FREE KIDS DANCE TRYOUT CLASS

STAR DANCE CENTRE LIABILITY AND MEDIA RELEASE FORM: the undersigned participant or parent/guardian of a minor, hereby voluntarily agree to the following terms and conditions: Acknowledgement of Risks: I acknowledge and fully understand that I (or the participant, if the participant is a minor) will be engaging in activities that may involve risk of serious injury, including permanent disability and death, property loss and severe social and economic losses at the Dance on Yonge event hosted by Canada National Dance Centre, operating as Star Dance Centre. Assumption of Risk: I agree that I (or the participant, if the participant is a minor) am voluntarily participating in these activities and using the premises and facilities with the knowledge of the danger involved. I hereby agree to expressly assume and accept any and all risks of injury or death. Waiver and Release of Liability: I hereby release, waive, discharge and agree not to sue the Canada National Dance Centre, operating as Star Dance Centre, its directors, officers, employees, and agents (hereinafter collectively called "the studio") from all liability to me, my children, my heirs, assigns and next of kin for any and all claims, causes of action, obligations, lawsuits, charges, complaints, controversies, damages, costs or expenses of whatsoever kind, nature, or description, whether direct or indirect, in law or in equity, in contract or in tort, or otherwise, whether known or unknown, arising out of or connected with my (or the participant's, if the participant is a minor) participation in these activities. Media Release: I hereby grant the studio the absolute right and permission to use, publish, broadcast, and copyright the photographic images, video or audio recordings that they have taken of me (or the participant, if a minor), or in which I may be included with others, to use, reuse, publish, and republish the same in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, and for any purpose whatsoever, for illustration, promotion, art, editorial, advertising, trade, or any other purpose whatsoever without restriction as to alteration; and to use my name in connection therewith if they so choose.This agreement shall be governed by and construed in accordance with the laws of Canada, without regard to its conflict of laws principles. This agreement contains the entire agreement between the parties and supersedes any other agreements, statements, or representations not contained herein.
