This consent applies for 1 year when the form is submitted but you can contact us at any time if you wish to change or withdraw any of the details of consent given here.

Consent for Reconciles' kids photo & video content

I, the undersigned, hereby consent to the recording of photos and videos of my child listed above during the specified event/activity. I understand that these images may be used for various purposes, including: Promotional materials (brochures, flyers, social media), Website content, News articles or press releases. I acknowledge that the images/videos may be used without name identification, but my child's name may be included in certain instances.
