I have agreed to a 10-month contract with Masebotsana Modeling Academy for a weekly class lasting 90 minutes. To cancel my contract, I must provide 30 days' notice and understand that fees remain due regardless of my attendance. I acknowledge that I am required to attend a minimum of 80% of the classes, which amounts to 8 months of classes throughout the year. I am allowed to participate in all Academy events, though I am responsible for my own expenses. I consent for my pictures and videos taken during the Modeling lessons to be shared on social media. Attendance at three annual photoshoots and the graduation ceremony is compulsory. I must inform the Academy of any modeling interests and cannot enter pageants without the Director's prior approval. In the event of clashing events, I must prioritize the Academy's events and cannot attend other modeling sessions while I am registered. Additionally, I cannot share details of my modeling activities without crediting or acknowledging the Academy (tagging and mentioning) and must submit contracts from agencies before accepting any offers. Should I win prize money from pageants, I am obligated to pay the Academy 10% of those earnings.

I declare that all particulars provided by me on this form are true and correct and I undertake to adhere to the rules, regulations and decisions of the Academy and to any of the amendments thereof which are applicable to students in general and/or the programme of study for which I am registered. I understand that no fees will be refunded to me once I have been accepted as a student, even if I subsequently withdraw from this programme.
