Parent Permission form for Field Trip Participation

Dear Parent or Legal Guardian:

Your child is eligible to participate in a school sponsored activity requiring transportation to a location away from the school building. This activity will take place under the guidance and supervision of employees from Saint Patrick Catholic School. A brief description of the activity follows:

Name of Event: Walk-a-thon Movie Reward

Destination: State Theater, London, Ohio

Designated Supervisor of Activity: Classroom teachers

Date and Time: January 31, 2025, 8:45 to 11:15

Method of Transportation: School Bus

Student and Chaperone Cost: No cost for students

What to Wear: This will be a part of our Catholic Schools Week Spirit Week.  Students are encouraged to wear their Saint Patrick spirit wear or school colors. Students may choose to wear their school uniform if they wish.

If you would like your child to participate in this event, please complete, sign, and return the following statement of consent and release liability. As parent or legal guardian, you remain fully responsible for any legal responsibility which may result from any personal action taken by said named student.

I hereby consent to participation by my child in the event described above. I understand that his event will take place away from the school grounds and that my child will be under the supervision of the designated school employee on the stated dates. I further consent to the conditions stated above on participation in this event, including the method of transportation.
