Your entry and presence on the event premises constitutes your consent to be photographed, filmed, and/or otherwise recorded and to the release, publication, exhibition, or reproduction of any and all recorded media of your appearance, voice, and name for any purpose whatsoever in perpetuity in connection with Northside Christian Church, including, by way of example only, use on websites, in social media, news and advertising. PARTICIPATION RELEASE:

By registering I, do hereby agree to the following terms:

-I acknowledge that Day Camp 2025 at Northside Christian Church involves activities that carry inherent risks, including, but not limited to, outdoor play, group games, and physical activities. I understand that despite all precautions, accidents can occur.
-I hereby release, indemnify, and hold harmless Northside Christian Church, its staff, volunteers, and any affiliates from any and all claims, injuries, damages, or losses that may arise during my participation in the Day Camp activities, whether or not caused by negligence or accident.
-In the event of an emergency, I authorize the staff of Northside Christian Church to seek medical treatment for me if necessary and agree to assume responsibility for any costs associated with such care. I understand that every effort will be made to contact me or my emergency contact before any medical treatment is provided.
