Welcome to the new EDNA Umpire/Staff Registration Form.
In order to complete this form you will need to have the following documents ready to be uploaded at the appropriate time

- Information from Netball Learning (accreditation if you have any, exam pass details, Foundation course details)

- A completed copy of a ATO Statement from Supplier form - ATO STATEMENT BY SUPPLIER FORM - When completing the form the Supplier is you as the umpire.

If you are 15 or OVER: You will need to complete these prior to registering!
Evidence that you have completed the following courses from Sports Integrity Australia
- To access the courses please use the links below:
Child Safeguarding in Sport Induction (sportintegrity.gov.au) And search for Chlid Safeguarding in Sport induction Course
- Play by the rules: Child Protection & Safeguarding
PBTRs - Child Protection and Safeguarding (sportintegrity.gov.au) and search for Child Protection and Safeguarding Course

- Have a Photo of a Working with Child Check (or equivilent) if you are 18 or turning 18

I hereby declare and advise Essendon & District Netball Association (EDNA) that I will be providing my services to EDNA as a hobby and on a casual basis.

Any remuneration payable to me for this service should not be regarded as subject to PAYG Withholding Tax deductions.

I also advise that I am not a registered GST Supplier, therefore GST is not applicable to any remuneration payable to me for providing this service.

I agree to advise Essendon & District Netball Association immediately if there are any changes to my details or to this arrangement.

EDNA is committed to making a safe, friendly and welcoming environment for all our members and guests.
As officials you play an important role within EDNA, for both members and other officials of the Association.

All umpires & staff 15 or over of Essendon & District Netball Association must complete the below Child protection courses and upload a completion certificate once completed.
