FINANCIAL ASSISTANCE PROGRAM REQUEST FORM
Faith Community Baptist Church is committed to assisting its members develop every aspect of their lives. We recognize that in addition to meeting the spiritual and educational needs that there are times when members need assistance with their financial needs. The Financial Assistance Program is in place to offer assistance to members who are faced with critical financial distress. However, it should be noted that the determination of assistance is based on three primary criteria.

1. Nature and urgency of need.

2. Availability of resources.

3. History of financial management.

Please be advised that if granted assistance the participants may be required to complete he Credit and Money Management workshop offered here at Faith.
