WAIVER
In consideration of my child being allowed to participate in any way in any program, event or activity (including but not limited to practices, camps and Open tournaments) sponsored or sanctioned by HOLDER GOALKEEPING("HGK Events"),
I the undersigned, acknowledge, appreciate, and agree that:
By signing this agreement, I acknowledge the risk of physical injury while participating in HGK Events.
I also know the contagious nature of COVID-19 and voluntarily assume any and all risk that my child, my family, and I may be exposed to or infected by COVID19 (and/or any other communicable disease) by attending the HGK Events and that such exposure or infection may result in personal injury, illness, permanent disability, or death. I understand that the risk of becoming exposed to or infected by COVID-19 at HGK Events may result from the actions, omissions, or negligence of myself and others, including, but not limited to, HGK agents, professional coaches, volunteers, and program participants and their families.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any and all injury to my child, my family, or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child may experience or incur in connection with my child’s, my family’s or my attendance at or participation in HGK Events (“Claims”).I, for myself and on behalf of my child, family, heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, HOLD HARMLESS, AND COVENANT NOT TO SUE HOLDER GOALKEEPING, its affiliates, officers, directors, officials, professional coaches, volunteers, agents and/or employees (RELEASEES), from any and all Claims, demands, losses, and liability arising out of or related to any ILLNESS, INJURY, DISABILITY OR DEATH I, my child, or family may suffer, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
As an Individual signing up for any HGK Events myself, I the undersigned also acknowledge, appreciate and agree that by signing this agreement I acknowledge all the above risks while participating.
