STARS Annual Update

Welcome to the STARS Continuous Quality Improvement (CQI) Annual Renewal via Jotform. In order to maintain your STARS level certificate, your program is required to submit:
- An Annual Renewal Self-Reflection. STARS Annual Update Self-Reflection
- Your program's STARS CQI plan showing all your quality progress made since your last application or renewal. STARS CQI Template
Annual Renewals are due between the years of your STARS Application (years 2 and 3 of your STARS certificate cycle) on the anniversary of your program’s STARS Application approval date.

When you submit your renewal:

The STARS Team reviews your submission as soon as we are able and will notified you via email that your renewal is approved.
