Hold Start Date* This field is required.Hold requests require a 48-hour notice and a one-time $25 hold fee. Upcoming payments scheduled within this period are still due and all pre-paid days will be available for practice when your hold lifts, the result may change your future payment due dates. Holds cannot be processed retroactively. A valid card must be on file to proceed as holds can only be applied to accounts without balances, therefore outstanding fees will be collected during the hold process. Confirmation emails will be sent when scheduling is complete.
