Parties. This Agreement is between Ashley Duncan (the “Artist”) and*, a living person, with a birthdate of   *  who lives at the following address:   *   (the “Client”, “You” or “Your”).
From time to time in this Agreement, each may be referred to as a “Party” or collectively, the “Parties”.

Appointment. Your tattoo appointment (“Appointment”) will take place on      at      at the Artist’s studio located at 3543 W Braddock Rd Suite C-6, Alexandria, VA 22302. Your tattoo may require multiple appointments to complete. Any such appointment will be subject to the terms of this Agreement.

  • Deposit. A deposit of $400 (the “Deposit”) must be paid along with this Agreement. When the Artist  receives this signed agreement, and the Deposit, the Artist will reserve the Appointment date and time  for You and forgo all other opportunities to work with other clients at that date and time. As such, the  Deposit will not be refundable under any circumstances.

If Your tattoo requires multiple appointments to complete, You will need to pay another deposit to secure any such appointment.

You understand and agree that the non-refundable nature of the Deposit does not constitute a penalty  and is reasonable, because it would otherwise be impracticable and extremely difficult to calculate the  actual damages resulting from any such lost opportunities.

Fees. You will be charged $200 per hour for the work done on Your tattoo.

All work will be charged in 15-minute increments and time will be rounded up to reflect the same. (Ex. If  Your tattoo takes two hours and 20 minutes to complete, You will be charged for two and a half hours of  work).

There is a two (2) hour minimum for each tattoo, so even if Your tattoo takes less than two hours, You will  be charged for a full two hours.

Payment. Payment will be due in full at the end of each Appointment. The Deposit will be applied toward  any balance due.

Payment Method. You may pay your balance by credit card, debit card, PayPal or cash. No other payment  methods will be accepted.

Chargebacks. If You are unhappy with this Agreement or any of the work performed under this Agreement,  You are welcome to pursue any of the remedies offered to You under this Agreement. If You initiate a  chargeback after payment of any properly accrued fee or deposit, You will be responsible for any fees  imposed on the Artist in connection with dealing with such a chargeback, including, but not limited to any  bank fees imposed on the Artist by the Artist's bank, and any fees, including reasonable attorneys' fees  incurred by the Artist for having to respond to such a chargeback.

Collection. If the Artist is required to pursue collection efforts to collect any fees due under this  Agreement, including any fees subject to a chargeback, You will be responsible for all costs of the same,  including, but not limited to, reasonable attorneys' fees and court costs.

Rescheduling. You may reschedule the Appointment up to two (2) times for any reason by providing the  Artist with notice at least one (1) week before the Appointment date. There will be no penalty for timely  rescheduling.

If notice is not timely given or You need to reschedule more than twice, You will need to pay a $400  Rescheduling Fee before another appointment date and time can be scheduled.

Client agrees and understands that when notice is not timely given, the Artist misses the opportunity to  fill that time slot and other clients may miss the opportunity to receive the Artist’s services, as such, the  Rescheduling Fee is not a penalty.

Lateness. You should arrive at Your Appointment at the specified Appointment time. If You will be late for  Your Appointment, please be sure to let the Artist know as soon as possible. If You arrive more than 15  minutes late, You will be charged a $100 Late Fee.

If You are more than 30 minutes late for Your Appointment, You will be considered a No-Show. Your  appointment will then be canceled, and You will need to pay an additional $400 Rescheduling Fee to  reschedule Your appointment.

Emergencies. The Artist understands that emergencies can happen at any time, so if You have a true  emergency and cannot give the Artist enough advanced notice or end up arriving late to Your  Appointment, the Artist, in the Artist’s sole discretion may try to accommodate You.

Please understand that while the Artist wants to be able to accommodate all emergencies, the Artist needs to be able to balance Your needs with the needs of the Artist’s business and other clients. As such, all  emergencies will be evaluated on a case-by-case basis and no accommodation is guaranteed.

Some examples include diabetes, blood disorders, heart problems, skin diseases, immune system  disorders, allergies, infections, or pregnancy.

Some medications that may affect your ability to get a tattoo include blood thinners, antibiotics, steroids,  immunosuppressants, anticoagulants, or Accutane.

Getting a tattoo if you’re pregnant or breastfeeding is not recommended because of the potential risks  involved. Aside from the various health risks, there is also a risk of affecting the quality of your tatoo due  to hormonal changes that may cause skin stretching, sagging, discoloration, fading, or distortion. AFTERCARE INSTRUCTIONS

AFTER GETTING YOUR ART PIECE, IT IS IMPORTANT TO TAKE GOOD CARE OF IT TO ENSURE PROPER  HEALING AND PREVENT INFECTION OR OTHER COMPLICATIONS. FOLLOW THESE STEPS TO CARE FOR  YOUR NEW TATTOO:

    • Leave the bandage on for at least 2 hours or as instructed by the Artist. Do not remove the  bandage until you are ready to wash your tattoo.

    • Wash your hands with soap and water before touching your tattoo. Gently remove the bandage  in the shower using lukewarm water. Use a mild soap (such as unscented antibacterial soap) and  your fingertips to gently wash away any blood, ink, or plasma from your tattoo. Do not scrub,  rub, or use a washcloth or sponge on your tattoo. Let your tattoo air dry. Do not use a cloth  towel or a hair dryer on your tattoo.

    • Apply a thin layer of unscented moisturizer (such as Aquaphor, Lubriderm, Vitamin A&D  Ointment) to your tattoo. Do not use petroleum products (such as Vaseline) or alcohol-based  products (such as rubbing alcohol) on your tattoo. These products can clog your pores, dry out  your skin, and interfere with the healing process. Do not over-apply the moisturizer or cover  your tattoo with plastic wrap or bandages. Your tattoo needs to breathe and heal naturally. This  also means your tattoo needs to be moisturized consistently to prevent excessive dryness while  airing out.

    • Repeat the washing and moisturizing process 2-3 times a day for the first week or until your  tattoo is healed. You may notice some scabbing, peeling, flaking, or itching during this time. This  is normal and part of the healing process. Do not pick, scratch, or peel off any scabs or flakes  from your tattoo. This can damage your skin, cause infection, and affect the quality of your  tattoo. If your tattoo itches, moisturizing your tattoo may help relieve the dryness and itch. You  can apply some ice or a cold compress to relieve it. You can also take some antihistamines (such  as Benadryl) to reduce the itching and swelling.

    • IMPORTANT: Avoid soaking, swimming, tanning, or exposing your tattoo to direct sunlight until it  is fully healed. These activities can irritate or damage your skin, cause infection, and fade or  distort your tattoo. If you need to go outside, cover your tattoo with loose clothing and apply  sunscreen (at least SPF 30) to protect it from the sun. If you need to shower or bathe, keep it  short and avoid hot water or steam. Do not submerge your tattoo in water for more than 10  minutes at a time.

    • Avoid wearing tight clothing or accessories that may rub against your tattoo until it is fully  healed. These items can irritate or damage your skin, cause infection, and affect the quality of  your tattoo. Wear loose and breathable clothing that allows air circulation around your tattoo.

    • Monitor your tattoo for any signs of infection or other complications such as redness, swelling,  pus, fever, rash, blisters, pain, or odor. If you notice any of these symptoms, contact the Artist or  seek medical attention as soon as possible. You may need antibiotics or other treatments to  treat the infection or complication.

    • The Artist may have specific recommendations based on the type and location of your tattoo and  your skin type and condition. If you have any questions or concerns about your tattoo or its  healing process, do not hesitate to contact her for advice.
