  • Client Terms of Agreements for Pet Grooming Services:

    1. Vaccination Requirements: For your pet's safety and that of our staff, we require up-to-date vaccination records (Dogs: Rabies, DHPP, and Bordetella; Cats: Rabies and FVRCP) before any grooming services are rendered.
    2. Use of Restraints: To ensure the safety of both your pet and our groomers, we may use humane restraints such as muzzles, elastic collars, slings, and straps as necessary. Please disclose if your pet has a history of biting.
    3. Grooming Process and Risks: We strive for a calm grooming environment, but accidents (like nicks from grooming tools) can occur if pets do not remain still. To minimize distractions, clients should not interact with their pets during grooming. If necessary for safety, we reserve the right to end a grooming session early, with the full grooming fee applied.
    4. Client Liability: Clients assume all liabilities for their pet's behavior and health. Onalaska Animal Hospital and our groomers are not responsible for damages, loss, or claims due to pre-existing condition of the pet. 
    5. Health and Allergy Disclosure: Please inform us of any allergies, sensitivities, or medical conditions your pet has so we can tailor our grooming approach. We also welcome information on any previous grooming experiences to avoid repetition of unpleasant experiences. 
    6. Sensitive Procedures: If certain grooming procedures like nail trimming or ear cleaning cause your pet distress, we will stop those procedures and recommend they be handled by a veterinarian if necessary.
    7. Emergency Authorization: In case of an emergency, you authorize our groomer/stylist to obtain necessary veterinary services at your expense, covering all conditions including advanced age, nervousness, or any medical condition. 
    8. Flea Treatment: If fleas are detected, we will administer a flea bath at an additional cost to ensure salon sanitation, using gentle and hypoallergenic products.
    9. Matted Coats:  De-matting is charged extra. Severe matting may necessitate shaving to protect your pet's health. We will not de-mat if it causes undue stress or pain to your pet. be aware that shaving can reveal or exacerbate skin conditions. 
    10. Custom Cuts:  Custom cuts or shave downs outside of breed-specific styles will be performed to the best of our understanding of your instructions, with no guarantee beyond our professional capability.
    11. Scheduling: Groomer appointments typically last 2 to 3 hours, subject to changes based on your pet's behavior or coat condition.
    12. Cancellations and No-Show Policy: A minimum of 24 business hours' notice is required for cancellations. Missed appointments incur a fee of 50% the grooming price, payable before that next grooming or veterinary services. Three no-shows may lead to servicer refusal. 
    13. Late Arrival:  Late Arrivals may face delays or appointment cancellation, with the no-show policy applied for arrivals 30 or more minutes late.
    14. Payments and Late Payments:  Payment is due at service time. Returned checks incur a $50 fee, payable before the next grooming or veterinary service. Additional acceptable forms of payment include Cash, Debit, Visa, Mastercard, and Discover. 
    15. Professional Discretion:  Our trained professionals may alter or cease grooming if it is deemed in your pet's best interest or if the pet shows aggressive behavior. We reserve the right to refuse service at any time.
    16. Elderly Pets:  Extra care is given to older pets, but we cannot be held responsible for stress reactions. Grooming Services on elderly or frail pets are provided at the client's risk, and we may modify or terminate the haircut if it causes too much stress. 
  • I, the undersigned, have read, understand, and agree to the above terms for the grooming of my pet at Onalaska Animal Hospital. I do herby entrust my pet to Onalaska Animal Hospital LLC, for the purposed of grooming my pet.
