By submitting this form, you confirm that you understand and agree:

• Completed Application is required prior to being allowed to book your first session.
• A video or in person meet  is required for identity verification of you being a real prior to the session.
• A deposit is required at time of submitting session request.
•  If not previously paid in full by day of session, the remaining 50% of the session cost  will be collected while you are on your knees before the start of the session.
• You agree that failure to meet these minimum requirements will result in the cancellation of your booking and you won't be allowed to book a future session without paying Full Session Price at time of booking.
