Welcome! This form is the first step in booking your one-of-a-kind photoshoot with Cristina Marie Photography LLC. By completing it, you'll help me organize your session, choose your desired collection, select your model release agreement, and request your preferred date. Once submitted, I’ll confirm my availability and send you the appropriate invoice and portrait agreement/contract for digital signature. To secure your session date with me, a 50% non-refundable retainer is required, with the remaining balance due 48 hours before your confirmed session date. Completing this form ensures a smooth and efficient booking process for your special portrait session. All sales final.

Model Release Agreement

To proceed with your booking, please carefully review the model release agreement below and select the best option that reflects your preferences regarding the use of images from your session with Cristina Marie Photography LLC.
Model Release: The Client grants permission to the Photographer and his/her legal representatives and assigns, the irrevocable and unrestricted right to use and publish photographs of the Client, the Client’s child or children, or which the Client may be included for editorial, trade, advertising and other purposes, and in any other manner and medium; to alter the same without restriction; and to copyright the same. The Client hereby releases the Photographer and her legal representatives and assigns from all claims and liability relating to said photographs. The Client hereby releases and forever discharges the Photographer from any and all claims and demands that may arise out of or in connection with the use of the photographs, including without limitation any and all claims for libel, violation of copyright laws, or violation of any right of publicity or privacy. This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, owners and assigns of the Photographer. The Client releases all claim to profits that may arise from use of the photographs. ** If the Client does not agree to the model release as stated, another choice may be selected from the options below.

(ADDITIONAL 20% FEE APPLIED TO TOTAL COLLECTION PRICE)Limited Privacy Agreement (MATERNITY AND MOTHERHOOD SESSIONS ONLY): I acknowledge and agree to a limited privacy agreement. Cristina will refrain from sharing images from my session on any social media platforms and agrees not to utilize images for any marketing, editorial, trade, and/or advertising materials that contain nudity, (or implied nudity), and breastfeeding without my permission. I have received examples and understand which images are included in the agreement. (NO ADDITIONAL FEE INCURRED)Limited Privacy Agreement (NEWBORN SESSION ONLY): I acknowledge and agree to a limited privacy agreement. Cristina will refrain from sharing family images and the use of the family/child's name from my session on any social media platforms and agrees not to utilize images for any marketing, editorial, trade, and/or advertising materials without my permission. I have received examples and understand which images are included in the agreement. (NO ADDITIONAL FEE INCURRED)
